Zusammenfassung Frauen mit Genitalcarcinomen zeigen häufig eine Vermehrung des roten Knochenmarkes im Femur. Die vorliegende Arbeit enthält Angaben über die Struktur des Knochenmarkes von Wirbelkörpern, Femur und Humerus bei 86 Frauen mit Carcinomen des Collum uteri, Corpus uteri bzw. der Ovarien. 47mal fand sich eine über das altersübliche Maß hinausgehende Ausdehnung des roten Markes im Femur. 34 dieser Fälle zeigten mikroskopisch eine gesteigerte Gesamtblutbildung und alle außer einem Fall eine Zunahme der Erythropoiese. In 39 der 47 Fälle waren die Sinus des Knochenmarkes hyperplastisch, so daß in der Mehrzahl der Fälle die rote Farbe des Markes nicht nur durch die Steigerung der Erythropoiese, sondern auch durch den vermehrten Erythrocytengehalt des Knochenmarkes hervorgerufen war. Zwischen dem Umfang der Erythropoiese im Wirbelund Femurmark bestand eine lineare Beziehung. Das gleiche galt für die Beziehungen zwischen der Leukopoiese im Wirbel- und im Femurmark. 70mal wurde eine Hämosiderose des Femurmarkes und/oder der Milz beobachtet. In 44 dieser 70 Fälle enthielten die Krankenblätter keine Angaben über eine vorausgegangene Behandlung mit Bluttransfusionen. — Die Ergebnisse werden im Lichte erythrokinetischer Untersuchungen anderer Autoren über die Erythropoiese und den Eisenumsatz bei Krebskranken diskutiert. Aus diesen Untersuchungen und aus den vorliegenden Ergebnissen wird geschlossen, daß die Erythropoiese durch die vermehrte Hämolyse und durch die Hypervolämie stimuliert wird. Die Hämosiderose bei fehlender Transfusionstherapie kann die Folge eines erhähten Eisenumsatzes sein und dient möglicherweise dazu, toxische Stoffwechselprodukte des Krebskranken zu entgiften. Schließlich stützen unsere Ergebnisse die Hypothese, daß die Hyperplasie des Femurmarkes nicht allein einen kompensatorischen Vorgang im engeren Sinne darstellt, sondern bereits zu einer Zeit auftritt, zu der im Wirbelmark noch Reserveraum für die weitere Blutbildung vorhanden ist.

Summary The red bone marrow within the femur is found to be increased in many cases of female genital cancer. The present work deals with the structure of bone marrow in the vertebral column, femur and humerus of 86 women who died from cancer of the collum uteri, corpus uteri and ovaries, respectively. Bone marrow of the femur was hyperplastic in 47 patients. Thirty-four of these microscopically had an increased hemopoiesis and except one, all of them showed an increased erythropoiesis. In 39 of the 47 patients the sinuses of the bone marrow were hyperplastic. Thus, in most cases the red of the marrow was caused not only by excessive erythropoiesis but also by an increased content of erythrocytes. A linear correlation was found between the degree of erythropoiesis in the femur and vertebral bone marrow. The same was true for the leukopoiesis. Seventy cases were characterized by an intense hemosiderosis of the femoral marrow and/or the spleen. Among them were 44 patients in whom no evidence for transfusion therapy could be obtained. The results are discussed in the light of erythrokinetic studies by former authors on erythropoiesis and iron turnover in cancer patients. From these studies and from the present results it is concluded that erythropoiesis is stimulated by increased hemolysis and by hypervolemia. Hemosiderosis in the absence of transfusion therapy may be the result of enhanced iron turnover and probably serves to counteract toxic metabolites of the cancer patient. Finally, our results provide evidence in favor of the hypothesis that hyperplasia of the femoral marrow does not represent a compensatory mechanism sensu strictori but occurs at a time when residual space for additional hemopoiesis in the vertebral marrow is not yet exhausted.
