Zusammenfassung Eine optimale chirurgische Erstversorgung am Unfallort und auf dem Transport ist in hohem Grade abhängig von den zur Verfügung stehenden Geräten, Instrumenten und Medikamenten. Seit vielen Jahren bemühen sich die verschiedensten Gremien, eine Mindestausrüstung auszuarbeiten und zusammenzustellen, die einmal in einem sog.Unfallkoffer und zum anderen in einemRettungswagen enthalten sein soll. Die chirurgische Erstversorgung am Unfallort und auf dem Transport ist sicher nur dann optimal, wenn ein einheitlicher nach einer DIN-Vorschrift ausgestatteter Notfallkoffer vorhanden ist. Bei entsprechenden personellen und finanziellen Möglichkeiten ist es zweckmäβig, den normalen nach DIN genormten Rettungswagen (RTW) zu erweitern. Nach den neuzeitlichen Forderungen, einen in der Notfallmedizin bewanderten Arzt zu den in ihren vitalen Funktionen gestörten Patienten heranzubringen, wurde das Rettungswagensystem weiter entwickelt und ausgebaut. Die Besatzung dieser Wagen besteht aus zwei speziell ausgebildeten Notfallsanitätern und einem Arzt. Dieser Rettungswagen, mancherorts auch Notarztwagen genannt, sollte an einer chirurgischen Klinik, wo das Personal entweder in der Unfallambulanz oder auf der Wachstation mit eingesetzt werden kann, stationiert werden. Mit Hilfe eines genormten Notfallkoffers, der in den Fahrzeugen von Polizei, Hilfsorganisationen und auch Ärzten nicht fehlen sollte, und bei vermehrtem Einsatz des mit einem Arzt besetzten Rettungswagens wird es möglich sein, die Zahl der am Unfallort und auf dem Transport sterbenden Verletzten wesentlich zu verringern.

Summary The attainment of optimal surgical emergency treatment at the location of an accident and during transportation of the casualty will largely depend upon the equipment, the instruments, and the drugs that are at hand. Several official agencies have been trying for several years to plan and assemble a minimum scale of accoutrement, needed for an emergency bag and for an accident ambulance. Surgical emergency treatment at the site of an accident and during the transportation of the casualty is optimal only if a uniform emergency bag, equipped in accordance with DIN recommendations, is available. Provided sufficient personnel and finance are to be had, the equipment of the normal, DIN standardized accident ambulance may usefully be expanded. The accident ambulance organization has now been developed further in accordance with the current concept of the necessity for a doctor experienced in casualty work to be brought to the patient with disordered vital functions. The personnel of the accident ambulance consists of two specially trained accident ambulance men and one doctor. This accident ambulance, sometimes also called emergency doctor ambulance, should be stationed at the surgical department of a hospital, so that its personnel may be used either for the accident ambulance or in the casualty reception ward. The number of deaths at the location of an accident and during removal to hospital will certainly be reduced if a standardized emergency bag were supplied, which should also be carried in police cars, cars of First Aid organizations, and by all doctors, and if the number of ambulance cars that carry a doctor were increased.
