Summary The distinctive features of gastroduodenal leiomyosarcomas are reviewed and 3 cases presented. The first patient had a large exophytic leiomyosarcoma of the duodenum; the other 2 had gastric tumors of the same histologic type. In retrospect, all 3 had clinical and roentgenologic findings which were highly suggestive of leiomyosarcoma. They were initially attributed to the more ubiquitous peptic ulcer diathesis, with a resultant delay in diagnosis and therapy. Even with the tumor exposed at the time of laparotomy, its true nature was somewhat obscure, corroborating the Hippocratic adage that others have applied to leiomyosarcomas: “Experience is fallacious and judgment difficult.”
