Summary In 1952 a special type of benign lymphadenitis was demonstrated, with a particular clinical feature and histological picture, which afterwards was given the name “Subacute lymphadenitis nuchalis et cervicalis PiringerKuchinka”. Now there are 62 patients who have been kept under observation up to until 8 years, women being more commonly affected than men, in the ratio of about 3 to 1, the average age of the total 30,8 years. A certain delicate constitutional type of increased susceptibility is prevalent. The microscopic picture of the lymph node is characterized by single or small groups of epitheloid reticulum cells scattered especially throughout the peripheric parts of the lymph node, but also the other cells must be checked carefully to exclude an early stage ofHodgkin's disease. In 49 of the patients the Sabin-Feldman-test and the complement-fixation-test for toxoplasmosis could be done afterwards, the results being positive in 46 (= 94%) cases, with typical titre curves in the more recent ones, those only some weeks or a few months after the beginning of illnes, and with remarkably high titres in the cases with illness not longer ago than one year. Since in a comparative group of normal persons of the same age the positive titre to be expected in this country is only 57%, the difference has a high statistic significance. Thus thetoxoplasmatic aetiology of the hitherto obscure lymphadenitis is proved, according toSiim's description and experimental identification of lymph gland swelling in toxoplasmosis acquisita. The complement-fixation-test with a new heat-stable dried antigen proved to be a very reliable method, its results agreeing in a high degree with those of the Sabin-Feldmantest.

Zusammenfassung Die 1952 als besonderes klinisches Krankheitsbild mit charakteristischem histologischem Befund vorgestellte, später von anderer Seite als „Subacute Lymphadenitis michalis et cervicalis Piringer-Kuchinka“ bezeichnete gutartige Lymphknotenaffektion umfaßt nunmehr ein Patientengut von insgesamt 62 Personen im Durchschnittsalter von 30,8 Jahren mit einer Beobachtungszeit bis zu 8 Jahren. Frauen sind etwa 3mal häufiger betroffen als Männer, und es herrscht ein bestimmter anfälliger Konstitutionstypus vor. Im histologischen Bild beeindruckt eine kleinherdige epitheloidzellige Proliferation besonders in den peripheren Lymphknotenanteilen, doch ist darüber das sonstige Zellbild nicht zu vernachlässigen, was besonders im Hinblick auf ein histologisch ähnliches Frühstadium des Lymphogranulom hervorzuheben ist. 49 Patienten wurden mittels Sabin-Feldman-Test und Komplementbindungsreaktion auf Toxoplasmose nachuntersucht, wobei 46 (= 94%) positiv reagierten und bei den erst wenige Wochen bis Monate zurückliegenden Erkrankungsfällen typische Titerkurven, bei jenen mit einer Krankheitsdauer unter 1Jahr auffällig hohe Titerwerte festgestellt werden konnten. Da bei einer Gruppe von Normalpersonen gleicher Altersverteilung nach der örtlich bestehenden Häufigkeit latenter Toxoplasmoseinfektionen nur 57 % positive Reaktionen zu erwarten gewesen wären, ergibt sich eine statistisch hoch signifikante Differenz. Damit erscheint der Beweis erbracht, daß es sich bei der bisher ätiologisch völlig unklaren Lymphknotenaffektion um eineLymphknotentoxoplasmose handelt, wie sie vonSiim beschrieben und tierexperimentell nachgewiesen wurde. Die Komplementbindungsreaktion mit einem neuen thermostabilen Trockenantigen erwies sich als sehr verläßliche Untersuchungsmethode und zeigte weitestgehende Übereinstimmung mit dem Sabin-Feldman-Test.
