    Beschreibung / Inhaltsverzeichnis: Résumé Les auteurs ont étudié la contamination bactérienne du liquide de drainage et des drains dans une série consécutive de 105 patients opérés d'une prothèse totale de hanche. Tous les patients ont été opérés dans une enceinte stérile de type Charnley Howorth Mark II, avec antibioprophylaxie systématique. Les prélèvements bactériologiques ont été réalisés au niveau de l'extrémité du drain, de sa lumière et au niveau de l'incision cutanée. Les cultures se sont révélées positives chez 27 patients, dont 21 fois au niveau de l'extrémité ou de la lumière du drain. Un staphylocoque coagulase négative a été retrouvé chez 15 patients, dont 12 fois au niveau de l'extrémité ou de la lumière du drain. Neuf patients avaient une culture positive au niveau de l'extrémité du drain et 16 au niveau de sa lumière. Quatre patients ont développé un hématome et un écoulement séreux. Aucun n'a présenté d'infection profonde pendant les 12 premiers mois après l'intervention. Cette étude confirme que les staphylocoques coagulase négative sont les germes les plus fréquemment rencontrés sur des prélèvements après prothèse totale de hanche. L'antibioprophylaxie doit en tenir compte.
    Notizen: Abstract Bacterial growth in relation to suction drains and suction drain tubes following total hip replacements was studied in 105 consecutive patients. All the patients were operated on in a Charnley Howorth Mark II sterile enclosure without walls, but with a body exhaust system and routine antimicrobial prophylaxis. Specimens for culture were taken from the suction drain tip, from inside the suction tube and the incision site. Positive cultures were found in a7 patients; in 20 of them from either the drain tip or the drain track. In 15 patients, the culture revealed coa-gulase-negative staphylococci; in 12 of them from the drain tip or the drain track. Eight patients had a positive culture from the tip of the drain and 16 from the drain track. Four patients had signs of wound hematoma and serous discharge. No patient developed signs of deep infection during the first 12 months after surgery. This study confirms that coagulase-negative staphylococci are the most common single species obtained from cultures after total hip replacement. Routine antimicrobial prophylaxis against coagulase-negative staphylococci might be considered.

    Notizen: Abstract The regional changes in quantities of the glial S-100 protein and the neuron specific enolase in the rat nervous system have been studied after long-term exposure to 2,5-hexanedione. The wet weights of most of the examined nervous tissues were found to be reduced, with an extensive effect seen in the brain stem. Using dot immunobinding assays, the concentrations of S-100 were found to be increased in most of the examined tissues, but unaffected in the brain stem. The total amount of S-100 per tissue was markedly reduced in the brain stem. The content of neuron specific enolase was reduced only in the brain stem. Thus the effects of 2,5-hexanedione on the nervous system varied regionally. The brain stem was severely atrophied with a reduction of neuronal as well as of glial marker proteins. Other brain regions contained increased glial cell marker proteins as signs of progressive astroglial reactions.

    Notizen: Abstract The most common graft in anterior cruciate ligament (ACL) surgery involves using the central one-third of the patellar tendon. Knowledge concerning the postoperative disability after harvesting the patellar tendon is, however, limited. The aim of this study was to evaluate the impact patellar tendon suture and bone grafting of the patellar bone defect might have in terms of functional outcome and patellofemoral pain after harvesting the bone-tendon-bone graft, compared with leaving the harvested site non-sutured and non-grafted. Sixty patients, scheduled for arthroscopically assisted ACL reconstruction, were randomly allocated to two groups. In group I, suture of the patellar tendon and bone grafting of the patellar defect were performed. In group II, the tendon gap and the patellar defect were left open. Preoperatively, there was no significant difference between the groups when comparing objective knee stability, as measured with a KT-1000 laxity meter, Lysholm score, Tegner activity level, IKDC score, or patellofemoral pain score. Both groups had a significantly improved Lysholm score at the 2-year follow-up, without any difference between them. Tegner's activity level was significantly lower at follow-up, compared with the pre-injury level in both groups. The patellofemoral pain score improved significantly after the reconstruction, without any difference between the groups. Ultrasonography did not reveal any difference between the groups in terms of healing of the tendon gap. This study revealed no differences in donor site morbidity, functional outcome, patellofemoral pain score or knee joint stability between the two treatment groups. The conclusion is that suture of the patellar tendon and bone grafting of the patellar defect do not improve the functional results or reduce donor site morbidity after arthroscopically assisted ACL.
