Zusammenfassung Bei 71 Patienten mit chronischer Nierenerkrankung verschiedener Stadien fand sich eine lineare enge Korrelation zwischen der Konzentration des „wahren Kreatinins“ im Serum und der des 3-Methyl-Histidins (r=0.79). In ersten orientierenden Untersuchungen wird wahrscheinlich gemacht, daß der Ausscheidungsmechanismus des 3-Methyl-Histidins sich von dem des Histidins grundsätzlich unterscheidet. Histidin wird glomerulär filtriert und 5–7% der filtrierten Menge erscheinen im Urin, 3-Methyl-Histidin wird filtriert und zusätzlich sezerniert. Die 3-Methyl-Histidin-Clearance liegt beim Gesunden dreimal höher als die endogene Kreatininclearance, bei starker Einschränkung der Nierenfunktion fällt sie unter den Wert der Kreatininclearance ab.

Summary In 71 patients with different stages of renal disease, a close linear correlation was found between the concentration of “true creatinine” in the serum and that of 3-methyl-histidine (r=0.79). First orientational investigations indicate that the excretion mechanism of 3-methyl-histidine is fundamentally different from that of histidine. Histidine is filtered glomerularly, and 5–7% of the filtered volume appears in the urine; 3-methyl-histidine is filtered and secreted as well. In healthy subjects, the clearance of 3-methyl-histidine is three times higher than the endogenous creatinine clearance, and it falls below the creatinine clearance when renal function is severely impaired.
