Zusammenfassung Ein neues Konservierungsgerät wird beschrieben. In ihm wurden 17 Hundenieren für 72 Std unter Hypothermie mit einer Humanalbuminlösung nichtpulsierend durchströmt und anschließend transplantiert. Sämtliche Nieren nahmen unmittelbar nach Transplantation ihre Funktion auf, die spätestens am 7. Tag pOp Normalwerte erreichte (Bestimmung der PAH und Inulin Clearance). Der hohe Grad der Sofortfunktion wird auf den niedrigen Perfusionsdruck (20 mm Hg) und auf die präzise Einstellung der Temperatur auf 7,5° C zurückgeführt. Während der Perfusion wurde der Substratverbrauch (UFS, 02), sowie die Ansammlung von Stoffwechselendprodukten (Ammoniak, Harnsäure) im Perfusat überprüft. Ihre Bedeutung für die Limitierung der Konservierungszeit wird diskutiert.

Summary A new preservation system is described. In this unit 17 dog kidneys were nonpulsatile perfused with hypothermic human albumin for 72 hrs and then transplanted. All kidneys started immediately to function and the PAH and inulin clearances attained normal values at least at the 7th postoperative day. The quality of the immediate function is referred to the applied perfusion pressure (20 mm Hg) and the applied temperature of the perfusion medium (7.5° C). During preservation substrate utilization (oxygen, unesterified fatty acids) and the accumulation of toxic metabolic products (ammonia, uric acid) were controlled in the perfusate. Their importance concerning to the limitation of preservation time is discussed.
