    Notes: Summary A spectrophotometric assay is described which, due to improved extraction conditions, allows quantitative determination of enzymatically active, non-zymogen acrosin, proacrosin and total acrosin activity from human sperm acrosomes. Acrosomal proteinase activity is assessed by acid extraction of the sperm pellet and the suspension medium before and after snap-freezing, followed by zymogen autoactivation. Release of acrosin from the acrosome can be used as a sensitive biochemical marker to characterize acrosomal membrane stability, severe disturbance of which may be the cause of impaired male fertility. Acrosin activities in different populations of semen specimens are reported and compared to data available in the literature. Different degrees of acrosomal membrane alterations are observed in men with oligozoospermia, teratozoospermia and polyzoospermia. Particularly in oligozoospermia, a significant increase of active, nonzymogen acrosin points to severe acrosomal membrane alterations and, in addition, to a premature activation of proacrosin, which may impair fertilization in certain individuals. Finally, acrosin activity is shown to be significantly influenced by the time of sexual abstinence. It is concluded that determination of acrosin may be a useful indicator of the fertility potential in men.

    Description / Table of Contents: Summary So far, only a few studies about the effects of nature cure on male infertility have been published. Most data are available for application of acupuncture in andrological patients. Since randomized, double-blind and placebo-controlled studies cannot be easily performed with acupuncture, it is difficult to interpret the possible effects on semen quality. Significant improvement in pregnancy rates and standard semen parameters after treatment with Chinese medical herbs has been reported in the Chinese scientific literature. However, almost all manuscripts are written in Chinese language or are not available in German libraries. Therefore, only English abstracts with insufficient data can be evaluated. Studies about homeopathic treatment of male infertility have not yet been performed. Most reports about positive effects on semen quality are based on unpublished observations and case reports.
    Notes: Zusammenfassung Bisher sind nur wenige nichtmedikamentöse und nichtschulmedizinische sowie naturheilkundliche Therapien bei männlicher Infertilität publiziert worden. Die meisten Daten stehen noch für die Anwendung von Akupunktur bei andrologischer Indikation zur Verfügung. Gerade hier sind placebokontrollierte oder doppelblinde Studien aber nur eingeschränkt möglich. Daher ist eine abschließende Beurteilung der teilweise bei Akupunktur beobachteten positiven Effekte auf die Ejakulatqualität nicht möglich. Beachtliche Behandlungserfolge mit Heilkräutern oder pflanzlichen Stoffen werden immer wieder in der chinesischen oder indischen Literatur berichtet. Die entsprechenden Artikel sind aber häufig in der Landessprache verfasst und in ihrer vollen Länge nicht einsehbar. Zudem sind die Produkte in Europa weder bekannt noch erhältlich. Berichte über Behandlungserfolge mit homöopathischer Therapie bei andrologischer Indikation finden sich in der wissenschaftlichen Literatur nicht. Auch kontrollierte Studien liegen nicht vor. Die homöopathische Literatur stützt sich hier offensichtlich vorwiegend auf Einzelberichte oder persönliche Erfahrungen.
