Zusammenfassung In einer prospektiven Studie an 50 Patienten mit akuter Pankreatitis wurden diagnostische Daten mit der Frage analysiert, ob Indikation und Zeitpunkt zum chirurgischen Vorgehen dadurch beeinflußt werden. Das initiale Labor und der initiale klinische Befund ermöglichen nicht, zwischen operierten und nicht operierten Patienten zu unterscheiden. Die Operationsindikation ist am besten durch wiederholte klinische Untersuchungen zu erkennen. Die initiale ERCP identifiziert mit hoher Sicherheit die biliäre Pankreatitis und leitet beim Gallengangstein direkt zur Therapie über.

Summary A prospective clinical study of 50 patients with acute pancreatitis was undertaken to estimate the significance of diagnostic data for assessment of the indication for and the timing of surgical intervention. Initial clinical and laboratory data did not facilitate the choice between conservative and operative treatment. Daily clinical assessment was the most helpful way to determine the indications for and the urgency of surgery. With the aid of the initial endoscopic retrograde cholangiopancreatogram, biliary pancreatitis could be accurately diagnosed and in cases of cholelithiasis, early surgery or endoscopic papillotomy could be performed.
