Zusammenfassung Die Rekonstruktion der Gesichtshaut durch Spalt- oder Vollhautdeckung oder Rollappenplastiken ergeben nur z. T. befriedigende Ergebnisse und hinterlassen häufig erhebliche Sekundärnarben an den Hebeorten. Da die mechanische Gewebeexpansion nicht nur die Haut dehnt, sondern die Zellteilung anzuregen scheint, kann hiermit ein Gewinn an gesunder Haut erzielt werden. Die Gewebeexpansion bietet daher die Möglichkeit, ortsständige, struktur- und pigmentgleiche Haut für Defektdeckungen heranzuziehen, ohne Sekundärnarben zu hinterlassen.Indikationen: grosse Nar ben oder Tumoren am Kopf, bei Alopecie und ausreichender behaarter Resthaut, ausgedehnte Gesichtsnarben in ausgewählten Fällen.

Summary Facial repair involving split-thickness, full-thickness grafts or tube pedicle flaps more often than not leads to unsatisfactory results, not to mention extensive scarring at the donor site. Mechanical tissue expansion not only stretches the skin, but seems to promote cell division in such a way that a surplus of healthy skin can be harvested. Tissue expansion thus enables the surgeon to employ adjacent skin of identical texture and pigment without secondary scars. The indications are large scars or tumors of the head, alopecia if there is sufficient residual hair-bearing skin, and certain types of extensive facial scarring.
