    Notes: Abstract Two previously healthy women are described who in their late thirties suffered transient strokelike episodes, consisting of initial headache and vomiting, with various subsequent neurological signs that were only partially reversible. Investigations revealed elevated serum creatine kinase, lactic acidosis, hypertriglyceridaemia, and ragged red fibres in the muscle biopsy specimens. In both patients in vitro studies were performed on intact muscle mitochondria and muscle homogenate. Only in one was a mitochondrial defect found, located at the level of coenzyme Q. We conclude that these patients suffered from adult-onset mitochondrial encephalopathy, lactic acidosis and strokelike episodes (MELAS syndrome). Although the syndrome is often associated with long-standing neurological multisystem disease from childhood onwards, it should also be suspected in adults with strokelike signs of otherwise unexplained origin.

    Description / Table of Contents: Summary A patient with the clinical features of pseudohypoparathyreoidism and elevated concentrations of serum CK and LDH, which normalized after successful therapy, is described. Clinical signs of myopathy did not exist. The bioptical material from the m. tibialis anterior was microscopically normal. The biochemical analysis revealed a reduced phosphorylase-a-activity with the total phosphorylase-activity (a and b) being within the normal range. The significance of these findings as well as possible pathogenetic mechanisms are discussed.
    Notes: Zusammenfassung Es wird über einen Fall von Pseudohypoparathyreoidismus berichtet, bei dem gleichzeitig erhöhte CPK- und LDH-Konzentrationen im Serum festgestellt wurden, die sich nach Therapie normalisierten. Klinische Zeichen einer Myopathie bestanden nicht. Das Biopsiematerial vom m. tibialis anterior war mikroskopisch unauffällig. Biochemisch fand sich eine verminderte Phosphorylase-a-Aktivität bei normaler Gesamtphosphorylase-Aktivität (a und b). Die Bedeutung des Befundes und die möglichen pathogenetischen Mechanismen werden diskutiert.

    Description / Table of Contents: ZusammenfaBung Aufgrund der Erfahrungen an 713 operierten Patienten mit artiellen Aneurysmen zwischen 1959 und 1976 wird demonstriert, daB zur chirurgischen Behandlung arterieller Aneurysmen an Aorta und peripheren Arterien einfache, neben schwierigen Verfahren ihren Platz haben. Die Eingriffe unterscheiden sich durch folgende Behandlung der arteriellen Strombahn: 1. AuBchaltung durch Ligatur der zuführenden Arterien ohne Strombahnwiederherstellung, 2. Abtragung des Aneurysma unter BelaBung der Strombahn, 3. AuBchaltung des Aneurysma und Strombahnwiederherstellung durch eine Umleitung (an der abdominothorakalen Aorta: „Umwandlungsverfahren”), 4. Resektion und Wiederherstellung der Strombahnkontinuität am Ort des Aneurysma. Daneben wird demonstriert, daB auch einfache, neben aufwendigen und invasiven Verfahren zur Diagnose eines Aneurysma führen können.
    Notes: Summary From experiences with 713 patients, operated because of arterial aneurysms between 1959 and 1976, it is demonstrated that simple and complicated techniques of surgical therapy may be neceBary for treatment of arterial aneurysms at the aorta and peripheral arteries. The procedures can be differentiated according to the following mode of reconstruction of the vascular continuity: 1) simple ligation of the arteries without reconstruction, 2) excision of the aneurysms leaving sufficient vascularture, 3) occlusion or resection of the aneurysms and vascular reconstruction by a bypaB, 4) resection and reconstruction of the vasculature at the place of the aneurysm. It is demonstrated further that simple as well as complicated and/or invasive techniques can lead to diagnosis of an aneurysm.
