    Notes: We examined 30 patients with an arthroscopic suture repair for anterior shoulder instability in a retrospective evaluation. The follow-up period ranged from 12 to 58 months with an average of 22 months. Arthroscopic suture repairs were done on 14 patients (acute group, average age 26.1 years) with acute detached glenoid labrum, confirmed on arthro-CT, within 10 days after the injury and on 16 patients (secondary group, average age 25 years) with chronic should dislocation. The evaluation according to the Rowe scale resulted in a mean score of 97.1 for the acute group, compared with 92.7 for the secondary group. In each group we found one recurrent dislocation, which in the acute group was due to an adequate trauma. Two of the 14 acute group patients showed a reduction in external rotation of up to 20°, compared with 6 patients in the secondary group. The external rotation of one patient in the secondary group was reduced to 40°. The isokinetic muscle strength was decreased in both groups, both for 60°/s and for 120°/s, to 85% compared with the healthy side. The primary surgical therapy of young patients (below 25 years) with an acute shoulder dislocation and a detached glenoid labrum is recommended owing to the lower redislocation rate, an overall shortened course of treatment and a trend to better postsurgical range of motion.

    Description / Table of Contents: Zusammenfassung Basilarisverschlüsse werden unter pathologischanatomischen und klinischen Gesichtspunkten betrachtet. Dabei werden sowohl Korrelationen als auch Diskrepanzen zwischen pathologisch-anatomischen Befunden, klinischen Befunden und Ergebnissen der apparativen Zusatzdiagnostik besonders beachtet. Die hieraus resultierenden Möglichkeiten diagnostischer und prognostischer Fehlbeurteilungen werden erörtert. Die Überlegungen führen zu der Schlußfolgerung, daß bereits klinisch (d. h. angiographisch) eine Einteilung in orale, mittlere und kaudale Basilarisverschlüsse versucht werden sollte.
    Notes: Summary Occlusions of the basilar artery are reviewed under pathologic-anatomical and clinical aspects. Special attention is devoted to correlations as well as discrepancies between pathologic-anatomical findings, clinical findings and the results of additional diagnostic procedures. The resulting possibilities of erroneous diagnostic and prognostic interpretations are discussed. The considerations lead to the conclusion that an attempt should be made to classify clinically the basilar artery occlusions into oral, median and caudal types. The present paper takes the pathologic-anatomical as well as the clinical findings into consideration. It has been reported in recent years that in some instances patients have survived basilar artery occlusions. Similar observations suggested the advisability of investigating the basilar artery occlusions among our patients with regard to pathogenetic, diagnostic and prognostic criteria.

    Notes: Abstract Maitotoxin (MTX) induces an increase of [Ca2+]i and of phosphoinositide breakdown in various cell types. The [Ca2+]i increase followed with fluorescent probes on cell suspensions has been described as slow and lasting, in contrast to the “signal” induced by calcium ionophores such as ionomycin. MTX effects have been studied on two fibroblastic cell lines, BHK21 C13 and FR 3T3, synchronized by serum deprivation treatment performed in an isoleucine-free medium for BHK21 C13 cells. In BHK21 C13 cells, flow cytometry analysis showed that two stages, G1/S and G2/M, were particularly susceptible to MTX treatment. Scanning laser cytometry demonstrated that calcium response of FR 3T3 fibroblasts followed with Indo-1 varied during the cell division cycle. The [Ca2+]i increase was almost always vertical, but its delay after MTX addition lasted from zero (S and G2/M transition) to 10–20 min (G1) or more (G2). No [Ca2+]i change could be detected during mitosis. The [Ca2+]i response at the S phase was biphasic. These observations suggest that (1) the lasting response described in the literature represents a global cell population effect, and (2) cells are more sensitive to MTX at specific stages of the cell division cycle, which could correspond to periods when calcium signals have been detected in different cell types.
