Zusammenfassung 52 Intensivpatienten mit einem SAPS von 12–15, die eine adjuvante CAVH erhielten, wurden retrospektiv nachuntersucht. Die negativ bilanzierte Gruppe A (n = 35) zeigte mit Beginn der CAVH eine Abnahme der Herzfrequenz, des ZVDs, der Pulmonalarteriendrucke, des Serumkreatinins und des FIO2 bei gleichzeitiger Steigerung des arteriellen Mitteldruckes (MAP). Trotz positiver Bilanz in Gruppe B (n = 17) konnten die gleichen Parameter stabil bzw. leicht gebessert werden. Im Hämofiltrat beider Gruppen konnten erhebliche Mengen an TXB2 und 6 Keto-PGF1 Alpha sowie 1,7 g Protein/Liter gefunden werden. Neben dem Flüssigkeitsentzug muss in beiden Gruppen die Toxinelimination als Ursache der kardiorespiratorischen Rekompensation angesehen werden.

Summary In 52 shock patients with a Simplified Acute Physiologic Score of 12–15, who were treated adjuvantly with CAVH, the hemodynamic and metabolic findings were reviewed retrospectively. In group A (n = 35) with a negative fluid balance, CAVH induced significant cardiorespiratory improvement. Group B (n = 17), with a positive cumulative fluid balance, showed stabilization of central venous pressure, pulmonary capillary wedge pressure, serum creatinine, and fractional inspiratory oxygen, a decrease in heart rate, and an increase in mean arterial pressure. Multiple simultaneous measurements of arachidonic acid metabolites in the ultrafiltrate showed a significant amount of TXB2 and 6 Keto-PGF1 Alpha as well as low-, middle- and high-weight proteins (1.7 g/l). Improvement in the cardiorespiratory variables in both groups suggests that CAVH in particular has the therapeutic potential to clear the bloodstream of vasoactive mediators.
