    Description / Table of Contents: Zusammenfassung Bericht über die Erfahrungen mit der Leksell-Drainage (Interventrikulostomie) bei 21 Patienten. Es werden die Einzelheiten des technischen Vorgehens und der diagnostischen Voraussetzungen besprochen und die Indikationen zur Anwendung gegeben. Diese besteht neben der „klassischen“ Indikation, d. h. einer Stenose oder bei einer Wiederherstellung des Aquädukts bei Prozessen nicht raumfordernder Natur auch noch bei einer zweiten Gruppe. Dort sollte bei Tumoren der hinteren Schädelgrube diese Methode angewandt werden, wenn man ein Rezidiv für möglich hält. Hier kann eine rechtzeitige Drainage spätere Operationen ersparen. Schließlich kann sie angewandt werden, um eine plötzliche Verlegung des Aquädukts nach Operation in den Fällen zu verhüten, wo sich ein Ödem nach Tumorentfernung in den rostralen Anteilen des 4. Ventrikels auswirken kann. Wenn man die Leksell-Drainage mit den üblichen internen Drainage-Verfahren vergleicht, so gibt diese eine größere funktionelle Sicherheit, die der der Torkildsen-Drainage gleichkommt.
    Notes: Summary Experience with the Leksell drainage (interventriculostomy) in 21 patients is reported. Apart from an account of the technical details and the diagnostic requirements, the indications are also discussed. Apart from the “classic” indication in a stenosis or restoration of the aqueduct in non—tumourous processes, we see a second reasonable field of application in subtotally resected posterior fossa tumours, when there is the possibility of a recurrence. In these cases the timely use of drainage can save a further operation. In addition, it can be used to forestall an immediate postoperative occlusion of the aqueduct due to oedema in cases in which a tumour was removed from the rostral part of the fourth ventricle. Compared to the common internal shunt procedures, the Leksell drainage shows a definitely greater functional security, equivalent to the Torkildsen drainage.

    Notes: Abstract Progressive neurological deterioration may occur after meningomyelocele repair. Magnetic resonance imaging almost invariably demonstrates a conus medullaris in an abnormally low position, whether neurological symptoms develop or not. Surgery of a secondary tethered cord is indicated when progression of neurological symptoms is documented. We performed a longitudinal study of posterior tibial nerve somatosensory evoked potentials (SSEPs) in children and adolescents after neonatal meningomyelocele repair. All patients were able to walk. Declining or negative posterior tibial nerve SSEPs were recorded in 15 patients; 14 of these had clinical signs of a secondary tethered cord. After surgery of the tethered cord, the SSEPs improved in 8 of 10 patients. Posterior tibial nerve SSEPs may contribute to the diagnosis of secondary tethered cord. After untethering, the evoked potentials demonstrate recovery of spinal cord function and might help to delineate prognosis.
