Zusammenfassung Von 1974–1976 wurden in unserer Klinik 83 offene Unterschenkelschaftbrüche stationär behandelt. 41 kamen primär zu uns, davon 50 % zweit- und drittgradig offen und 42 sekundär, wobei in 22 Fällen eine Osteomyelitis die Ursache war. In 19 Fällen war die Verletzung so schwer, daß die Behandlung als Erhaltungsversuch bezeichnet werden muß, darunter 16 Osteomyelitiden. Die Ergebnisse zeigen 11 Brüche knöchern ausgeheilt und stabil, davon 2 ohne und 9 mit Fehlstellungen oder orthopädischen Hilfsmitteln, 3 knöchern fragil mit Gehapparat und 4 Amputationen wegen nicht beherrschbarer Infekte. Die therapeutischen Schwierigkeiten werden aufgezeigt.

Summary From 1974–1976 83 fractures of the tibial shaft were treated in inpatients. 41 were admitted directly, 50 % with compound fractures grade II and III and 42 were referred with 22 cases of osteomyelitis. In 19 cases the injury was so severe, that the treatment had to be classified as attempt of conservation — 3 noninfected fractures grade III and 16 cases of osteomyelitis. The results were: 11 fractures with satisfactory bony union, 9 of them with deformities or orthetic devices, 3 fractures with unsatisfactory union and orthetic devices and 4 amputations because of incurable infection. The therapeutic problems are demonstrated.
