Zusammenfassung 0,5-3 % der Aortencoarctationen liegen nicht am Isthmus, sondem an atypischer Stelle im Bereich der thorakalen oder abdominalen Aorta. Leitsymptom der suprarenalen Stenosen ist die meist juvenile Hypertonie. Von 17 in den Jahren 1968–78 gefäßchimrgisch behandelten Patienten litten präoperativ alle an einer schweren Hypertension. 1mal wurde eine Erweiterungsplastik mit einem Dacronpatch, 16mal ein aortoaortaler Bypass gegebenenfalls mit Anschluß der Nierenarterien durchgeführt. Die Spätprognose der operierten Patienten ist günstig. Nach Literaturangaben weisen 74 % der gefäßchirurgisch Behandelten nach durchschnittlich 4 Jahren einen normalisierten und 15 % einen gebesserten Blutdruck auf. Im eigenen Krankengut waren 14 von 17 Patienten nach einer mittleren Nachbeobachtungszeit von 8,5 Jahren normoton; bei 3 Patienten war der Blutdruck gebessert (normoton mit Antihypertensiva).

Summary Between 0.5 % and 3 % of coarctations of the aorta are non-isthmic and cause mainly juvenile hypertension. From 1968 to 1973 at the Surgical Hospital of the University of Cologne and from 1973 to 1978 at the Surgical Hospital of the University in Munich, 17 patients with atypical hypoplasia of the descending aorta were operated on: 1 by patch graft, 16 by aorto-aortic bypass with — if necessary — revascularisation of the kidneys. All patients suffered from severe hypertension preoperatively. Follow-up ranged from 5 to 15 years. Late prognosis of vascular surgery is good. According to the world literature up to 1982 74 % of the patients treated surgically had normal blood pressure (bp) after an average of 4 years; 15 % had lowered bp. Of our own 17 patients 14 had normal by 5–15 years after reconstructive surgery; 3 patients had improved by (normotonic with drugs).
