Zusammenfassung Über die klinischen Erfahrungen und die Lungenfunktion bei Patienten mit idiopathischer Lungenfibrose wird berichtet; die Patienten wurde mit drei Kombinationen behandelt, nämlich Kortikosteroide mit entweder Azathioprin, D-Penicillamin oder K-Paraaminobenzoat. Die Vitalkapazität konnte durch die Therapiekombination mit Azathioprin häufiger verbessert werden als durch die übrigen Kombinationen, während die Kombination mit D-Penicillamin eher eine Verbesserung der Blutgase (in Ruhe und unter Belastung) bewirkte. — Die Kombination mit K-Paraaminobenzoat kann noch nicht beurteilt werden, da die Anzahl der Patienten zu klein und die Behandlungszeit zu kurz ist; die Befunde deuten jedoch an, daß diese Therapiekombination ebenfalls wirksam ist und insbesondere infrage kommt, wenn die beiden übrigen Therapiekombinationen nicht anwendbar sind. Allgemeine Empfehlungen für die Therapie der idiopathischen Lungenfibrose werden gegeben.

Summary Clinical experiences and physiological observations in lung function have been reported in patients with idiopathic pulmonary fibrosis. These patients were treated with three different therapeutical combinations (corticosteroids with Azathioprine, D-Penicillamine, or K-Para-aminobenzoate, respectively). Vital capacity is improved to a greater degree with Azathioprine and corticosteroids than with other combinations. D-Penicillamine seems to be superior with regard to blood gases when drawn at rest or under exercise. The number of patients treated with K-Para-aminobenzoate is too small and the time of treatment is too short to allow a definite assessment of this combination. The findings reported, however, indicate that corticosteroids and K-Para-aminobenzoate may prove to be an alternative when treatment with one of the other combinations is not possible in a patient. General recommendations for the therapy of idiopathic pulmonary fibrosis are given.
