    Notes: Summary In this study of more than 250 cases of retromuscular expander implantations after mammary amputations, we came to the conclusion that primary implantation is now just as successful as secondary implantation. The major problem is infection which occurred mostly in expander cases with a distant valve. As a result of the experience gained in this study, tissue expansion is considered to be the best method for immediate breast reconstruction. The skin-muscle-flap will be studied in a future series.

    Description / Table of Contents: Zusammenfassung Patienten mit einer angeborenen beidseitigen Facialis-Parese (Moebius-Syndrom) oder einer erworbenen (Zentrale Neurofibromatose) sind psychisch belastet durch das leblose, ausdruckslose Gesicht und physisch durch den Verlust der Sphincteren-Funktion an Augen und Mund. Der Lagophthalmus gefährdet die Corneae, und die bewegungslosen Lippen erschweren die Nahrungsaufnahme und das Sprechen. Die verlorene Funktion des M.orbicularis occuli kann durch die Implantation von Lidmagneten teilwiederhergestellt werden, während im schlaffen Mund Schleimhautausschneidungen und die periorale Fascia-lata Streifenimplantation helfen.
    Notes: Summary Patients with congenital bilateral paralysis of the facial nerve ( Mcebius syndrome) or acquired lesions (central neurofibromatosis) are emotionally handicapped by an expressionless face, as well as physically afflicted by the loss of sphincteric functions around the eyes and the mouth. Lagophthalmus endangers the cornea and the lifeless lips cause problems in eating and speaking. The lost function of the orbicularis occuli muscle can be partially restored through the implantation of magnets, while the drooping mouth can be improved by excision of mucosa and perioral implantation of fascia lata strips.
