Zusammenfassung Bei beidseitigen Lungenerkrankungen war bisher die operative Therapie nur in Ausnahmefällen möglich. Die simultane bilaterale Thoracotomie bzw. die bilaterale Thoracotomie mit entsprechendem Zeitintervall konnte nur jüngeren Patienten oder Patienten in sehr gutem Allgemeinzustand zugemutet werden. Die Einführung der medianen Stemotomie in die OP.-Strategie hat die Behandlungsmöglichkeiten wesentlich erweitert. Berichtet wird über 95 Sternotomien bei 86 Patienten. Indikationen, Art der durchgeführten Eingriffe und Komplikationen werden vorgestellt.

Summary To date, operative therapy has only been possible in exceptional cases of bilateral pulmonary disease. Simultaneous bilateral thoracotomy or bilateral thoracotomy with an appropriate time interval could only be undertaken in younger patients or in very in those good general condition. The introduction of median sternotomy into operative therapy has considerably extended the range of possible treatments. Ninety-five sternotomies are reported in 86 patients. The indications, types of procedures and the complications are presented.
