    Description / Table of Contents: Zusammenfassung. Die unklassifizierbaren Analfisteln stellen ein Problem der Fistelchirurgie dar. Von 1993 bis 1996 wurden 823 Patienten an einem analen Fistelleiden operiert. Ergebnisse: Eine Klassifikation nach dem Parks-Schema war bei 38 Patienten bei Unversehrtheit des Analkanals ohne Hinweis auf eine innere Fistelöffnung nicht möglich. Dies entspricht einer Rate von 4,5 %. Alle Patienten waren zum Teil mehrfach voroperiert. Nach alleiniger Fistelgangexcision heilten 53 % der Fisteln ab. 47 % rezidivierten. Bei nochmaliger Revision war durch Exploration des Intersphincterraums eine Nachklassifizierung der Analfisteln in der Hälfte der Fälle möglich. Bei diesen Patienten konnte eine kontinente Fistulektomie das Fistelleiden zur Abheilung bringen. Schlußfolgerung: Unklassifizierbare Analfisteln, bei denen die innere Fistelöffnung nicht auffindbar ist, können zunächst einer alleinigen Fistelgangexcision unterzogen werden. Im Falle eines Rezidivs sollten die Patienten einer Exploration des Intersphincterraums im vermuteten Gebiet des kryptoglandulären Infektionsorts unterzogen werden.
    Notes: Summary. In the surgery of anal fistulae, very demanding problems warranting special consideration are caused by the non-classifiable fistulae in ano. Results: Of 823 patients who underwent surgery for anal fistulae between 1993 and 1996, 38 (4.5 %) were, according to Parks' classification, non-classifiable; the anal canal was intact. There was no internal opening. All patients had already undergone operations, some of them multiple. In 53 %, complete healing of the fistula was achieved by using a single excision. In 47 % a recurrence developed. During a second revision we explored the intersphincteric space and were able to reclassify the fistulae in 50 % of the cases. A continent fistulectomy led to complete healing in these patients. Conclusion: Non-classifiable fistulae in ano, in which an internal opening of the fistula cannot be found, can primarily be treated by a single excision of the fistula. If recurrence does occur, the patient should undergo exploration of the intersphincteric space in the region, where the cryptoglandular infection is suspected.

    Notes: Based on the premise of achieving blood compatibility through mimicking the chemical constituents of the biologically inert surface of the unactivated platelet membrane, a process was developed that entails the covalent grafting of modified phosphatidylcholine molecules to materials including silica, polypropylene, and polytetrafluoroethylene (PTFE) polymer films. These materials were characterized using x-ray photoelectron spectroscopy (XPS) and contact-angle measurements. The phosphatidylcholine-containing materials (PC materials) were used as substrates in the platelet-adhesion assays and were subjected to enzymatic degradation evaluation. Phosphatidylcholine-grafted silica materials do not support platelet adhesion. In addition the number of adherent platelets correlate with the amount of grafted phospholipid present, as indicated by the phosphorus/carbon ratio obtained by XPS analysis. Platelet adhesion to phosphatidylcholine-grafted polypropylene and PTFE was inhibited 80% and 90%, respectively, when compared with platelet adhesion to unmodified polypropylene and PTFE. © 1996 John Wiley & Sons, Inc.
