    Description / Table of Contents: Summary Color-coded duplex sonography is being increasingly used in the head and neck as another method for diagnosis of such disorders as stenosis of arteries or veins or hemangiomas. A possible additional advantage of testing is the ability to differentiate benign from malignant tumors. Since the underlying cause of a suspicious lesion is most important for treatment strategies, we investigated histologically-proven benign and malignant cervical lymph nodes for determining the value of color-coded duplex sonography. In the present study, 135 patients (45 women and 90 men; ages 30–63 years) with palpable cervical lymph nodes were examined with both ultrasound and color-coded duplex sonography. Afterwards, lymph nodes were removed by open biopsies or neck dissections. The histological results were then compared to perfusion velocities and pulsation indices from the color-coded duplex sonography. However, no significant differences were found between the benign and malignant nodes. When comparing perfusion and color, each group was seen to be non-homogeneous and no typical distribution of vascularization was demonstrable. Findings showed that color-coded duplex sonography is currently not an accurate indicator of lymph node pathology.
    Notes: Zusammenfassung Hintergrund: Eine sichere Beurteilung der Gewebeerkrankung von Halslymphknotenschwellungen ist mit Hilfe der B-Scan-Sonographie nicht möglich. Mit der farbkodierten Duplexsonographie steht heute eine Untersuchungsmethode zur Verfügung, die Aussagen über die Echogenität sowie die Gefäßversorgung der untersuchten Areale liefert. Im Rahmen einer prospektiven Studie sollte geklärt werden, ob farbdopplersonographische Befunde Rückschlüsse auf die Dignität von Halslymphknoten erlauben. Patienten: Untersucht wurden 135 nicht therapierte Patienten (45 Frauen, 90 Männer, Alter 30–63 Jahre) mit Halslymphknotenschwellungen bislang ungeklärter Histologie. Methoden: Eingang in die Studie fanden Halslymphknoten, die sowohl palpatorisch (um eine spätere Identifikation im Neck-Präparat zu ermöglichen) als auch im B-Scan sonographisch nachweisbar waren und anschließend operativ entfernt wurden. Alle Patienten wurden präoperativ zuerst B-scan-sonographisch beurteilt. Sodann erfolgte die farbdopplersonographische Untersuchung. Die sonographischen und farbdopplersonographischen Befunde wurden dem histologischen Untersuchungsergebnis gegenübergestellt. Ergebnisse: Vergleichend ausgewertet wurden 80 Patienten mit histologisch gesicherten Plattenepithelkarzinommetastasen und 55 Patienten mit histologisch verifizierten akuten bzw. chronischen Lymphadenitiden. Hinsichtlich der Messung von Pulsatilitätsindizes imponierte kein signifikanter Unterschied zwischen benignen und malignen Knoten. Plattenepithelkarzinommetastasen und Lymphadenitiden zeigten bezüglich der qualitativen Farbverteilungsspektren lediglich unspezifische Vaskularisationsmuster. Schlußfolgerung: Die farbkodierte Duplexsonographie in der HNO-Heilkunde ist zum gegenwärtigen Zeitpunkt nicht geeignet, quantifizierbare Hinweise zur Dignität und/oder Spezifität von Halslymphknoten zu geben.

    Description / Table of Contents: Zusammenfassung Bei 82 Carcinompatienten wurden der Ernährungszustand und die celluläre Immunreaktivität vor einem chirurgischen Eingriff erhoben und mit der postoperativen Komplikationsrate verglichen. Bei 47 Patienten erfolgte die Bestimmung des Ernährungszustandes unmittelbar bei der Aufnahme (ambulant) mittels anthropometrischer Methoden (Körpergewicht, Gewichtsabnahme, Oberarmumfang, Tricepshautfaltendicke) und laborchemischer Parameter (Albumin, Präalbumin, retinolbindendes Protein, Transferrin, Cholinesterase). Bei 35 Patienten wurde der Eiweißkatabolismus (akut gestörte Proteinhomöostase in Richtung Stickstoffverlust) mit der Harnstoffproduktionsrate bestimmt. Die Bestimmung des Immunstatus erfolgte durch Applikation von 5 intracutan applizierten Antigenen und durch Bestimmung der Lymphocytenzahl. 55 % der Carcinompatienten hatten Anzeichen einer Mangelernährung. Kurative Eingriffe waren bei 17,4 % der mangelernährten und bei 50 % der normalernährten und übergewichtigen Patienten möglich (P 〈 0.0001). Die Komplikationsrate war bei den mangelernährten Patienten mit 47 gegenüber 20 % bei den normal ernährten signifikant erhöht (P 〈 0.05). Bei Carcinompatienten mit Mangelernährung und Anergie war kein kurativer Eingriff möglich. Auf Grund des vergrößerten postoperativen Risikos der mangelernährten Patienten sollten diese vor einer chirurgischen Behandlung erfaßt und einer Ernährungstherapie zugeführt werden.
    Notes: Summary The nutritional status and skin reactivity of 82 cancer patients were determined before surgery and compared with the postoperative complication rate. The nutritional status of 47 patients was evaluated by weight, height, weight-loss, arm muscle circumference, triceps skin-fold measurements, serum albumin, pre-albumin, retinolbinding protein, tranferrin, and cholinesterase. In 35 patients protein catabolism was assessed by the urea production rate (catabolism 〉 15 g/d). Immunity was assessed by the total lymphocyte count and a skin reactivity test. Using these criteria, 55 % of the patients were malnourished. Curative operations could only be carried out in 17.4% of the malnourished, but in 50 % of the normally nourished patients (P 〈 0.0001). Postoperative complications were increased in malnourished patients (47%) when compared with normaly nourished patients (20 %,P 〈 0.05). In anergic and malnourished cancer patients no curative surgical treatment was possible. Due to the increased postoperative complication rate in malnourished cancer patients, nutritional assessment, including the determination of cellular immunity should be performed after admission.

    Notes: The neutral fraction of medium volatility from raspberry oil has been analysed by gas chromatography and mass spectrometry. From the large number of constituents thirtynine have been identified, 24 of them for the first time, including dihydro-β-ionone (I), epoxy-β-ionone (II), damascenone (III)1, theaspirane (IV) and 2-hexen-4-olide (V). According to these results raspberry oil consists mainly of alcohols, ketones, aldehydes and lactones.
