    Notes: Abstract Visual grading, a laser Doppler flowmeter, a spectroradiometer, a two-channel erythema meter and a Minolta chroma meter were compared in the measurement of erythemas arising from immediate contact reactions produced either by 250 mM benzoic acid or 10 mM methyl nicotinate in petrolatum in an open application test or by ultraviolet irradiation. A good correlation between visual grading and objectively measured values was found for all the instruments, buth the laser Doppler flowmeter gave proportionally lower values for the ultraviolet erythemas than the skin reflectance meters, suggesting that these differ from erythemas induced by benzoic acid or methyl nicotinate. The laser Doppler flowmeter gave less repeatable results than other meters when measuring moderate and pronounced erythemas produced by ultraviolet irradiation. Measuring both the blood flow and the actual erythema may give more information about the reaction than either measurement system alone.

    Notes: Summary In the present study we examined the effects of chlorpheniramine and ranitidine, indomethacin, BW755C (an inhibitor of cyclo-oxygenase and lipooxygenase enzymes of arachidonic acid metabolism), dexamethasone, and capsaicin on nonimmunologic contact urticaria (NICU) induced in the guinea pig ear by benzoic acid, cinnamic acid, cinnamic aldehyde, methyl nicotinate, diethyl fumarate, or dimethyl sulfoxide. The intensity of edema in the urticarial reaction was quantified by measuring the ear thickness. Antihistamines inhibited reactions to intradermal histamine but not to agents causing NICU. Indomethacin and dexamethasone inhibited reactions to cinnamic acid and cinnamic aldehyde but not to other NICU agents. BW 755C and capsaicin had no effect on reactions to any of the NICU agents. Mast cell degranulation during the reaction was not seen in histologic sections. Histamine and capsaicin-sensitive nerves did not seem to be essential for the development of NICU in the guinea pig ear. The details of the inhibitory effects of indomethacin and dexamethasone are not clear, but it seems probable that more than one mechanism is involved in NICU due to different agents.

The aminotetralin derivative (+)-UH232 is a dopamine-receptor antagonist with complex pharmacological properties, including a 4 : 1 selectivity for the D3 vs. D2 receptor and a preference for the autoreceptors. Its behavioral profile differs markedly from that of other dopamine antagonists in exhibiting both stimulant and inhibitory features. In an effort to elucidate the role of different dopamine receptor subtypes in psychosis, we administered (+)-UH232 to drug-free schizophrenic patients. Six patients received single doses of (+)-UH232 over a dose range of 80 to 180 mg in a rising-dose, double-blind placebo-controlled design. Efficacy and safety were assessed over 8 hours after a single dose. In none of the patients at any of the doses was there an indication of a symptomatic psychosis improvement in response to (+)-UH232. On the contrary, an examination of individual cases revealed symptomatic worsening, such as increases in unusual thought content, anxiety, activation and hostility in four patients. No extrapyramidal movements were noted. Safety assessments were benign. These preliminary data suggest that putative dopamine D3 antagonism, in combination with preferential autoreceptor antagonism, does not alleviate but rather tends to worsen psychosis, at least following a single-dose regimen. However, the possibility cannot be excluded that a 5-HT2- receptor agonistic action of (+)-UH232, suggested by some animal data, has played a role in this treatment outcome. Replication with more selective agents and multiple dose regimens is necessary.
