    Description / Table of Contents: Zusammenfassung Bericht über 91 Patienten mit colorectaler Perforation der Jahre 1981 bis 1985. Aetiologie: Diverticulitis (45,1%); Carcinom (28,6%); chronisch-entzündliche bzw. ischämische Colitiden (11%). Operative Taktik bei Diverticulitiden: Einzeitige Resektion (15 von 41 = 36,6%); zweizeitig (5 von 41 = 12,2%). Diskontinuitätsresektion (16 von 41 = 39,0%). Bei den Carcinomen muß wegen der fortgeschrittenen Tumorstadien oft auf strenge onkologische Regeln verzichtet werden. Letalität 19,8%. Durch die konsequent durchgeführte primäre Resektion konnte die Letalität bei den Diverticulitiden niedrig gehalten werden (3 von 41 = 7,3%).
    Notes: Summary From 1981 to 1985, 91 patients underwent laparotomy because of colorectal perforation. Diverticulitis (45.1%), carcinoma (28.6%), inflammatory bowel disease, and ischemic colitis (11%) were the main etiological factors. The operative procedures for diverticulitis were: primary resection (15 of 41 = 36.6%) a two-stage procedure (5 of 41 = 12.2%), Hartmann's operation (16 of 41 = 39.0%). In colorectal carcinomas, the oncological rules often cannot be strictly adhered to because of the poor general condition of the patients. Total mortality was 19.8%. The low mortality in diverticulitis was due to consistent primary resection (3 of 41 = 7.3 %).

    Description / Table of Contents: Zusammenfassung In den Jahren 1982-1986 wurden bei 913 colorectalen Eingriffen in 165 Fällen die Kontinuität mit einem Klammernahtgerät wiederhergestellt (u. a. 116 Rectum- und Sigmacarcinome, 31 Diverticulitiden). Komplikationsrate von 20,3% versus 24% bei manueller Naht. Neun Nahtinsuffizienzen: dreimal Relaparotomie erforderlich, ein Patient verstorben. Anastomosenstenosen bei 5 Patienten — 1 Nachresektion. Technische Fehler bei 5 Patienten, davon nur einer durch das Gerät bedingt. Die locoregionäre Rezidivrate nach anteriorer Rectumresektion bei Patienten, deren Operation länger als 2 Jahre zurücklag, betrug 10,4% versus 17,8% bei Handnaht.
    Notes: Summary From 1982 to 1986, in 165 colorectal anastomoses we used a circular stapling device, the EEA Autosuture (e.g., 116 carcinomas of the rectum or sigma, 31 cases of diverticular disease). The complication rate was 20.3% versus 24% in hand-sutured anastomoses. Anastomotic dehiscence occurred in 9 cases: 3 patients required reoperation; 1 died. In 5 cases of stenosis, only 1 patient required further resection. Only one of five technical mistakes was caused by dysfunction of the EEA stapler; the others were the results of surgeon error. The local recurrence rate after anterior resection was 10.4% versus 17.8% in the hand-sutured group.
