    Description / Table of Contents: Zusammenfassung Die intraarterielle regionale Chemotherapie der Leber mittels implantierbarer Pumpen und Portsysteme wird bisher vorwiegend bei Absiedlungen colorectaler Carcinome durchgeführt. Die Behandlung und die Beurteilung der Ergebnisse muß folgende Faktoren berücksichtigen: Natürlicher Verlauf der Erkrankung; Klassifikation und Sicherung des isolierten Leberbefalls; Gefäßversorgung, optimale Katheterimplantation, regelrechte Perfusion; Medikamentenwahl und Dosierung; technische Komplikationen und lokale Toxizität; extrahepatische Metastasen und die Definition des Therapieerfolges. Es wird über die eigenen Erfahrungen bei 145 Patienten mit Lebermetastasen und 9 Patienten mit einem primär hepatocellulären Carcinom berichtet. Unter verschiedenen Therapiemodalitäten findet sich eine signifikant erhöhte Ansprechrate und derzeit eine echte Lebenszeitverlängerung von etwa 8 Monaten. Weitere prospektive Studien sind dringend erforderlich. Der Einsatz der regionalen Chemotherapie zur Behandlung von Lebermetastasen erscheint zum gegenwärtigen Zeitpunkt außerhalb von derartigen Studien nicht gerechtfertigt.
    Notes: Summary Intruarterial regional chemotherapy of the liver by implantable pumps and port-systems is currently applied predominantly in patients with isolated colorectal metastases. Therapy and evaluation of results must consider the following factors: natural course of the disease; classification of tumor extent in the liver; evidence of isolated liver metastases; vascular anatomy, optimal catheter-implantation technique, complete perfusion; choice of drugs and dosage; technical complications; local toxicity, extrahepatic recurrence, definition of therapeutic success. Our own experiences with regional chemotherapy in 145 patients with metastases to the liver and in 9 patients with hepatocellular carcinoma are reported. Using different therapeutic modalities we found a significantly enlarged response rate and at this time a true prolongation of life of about 8 months. Further prospective studies are necessary. The use of regional chemotherapy for liver metastases seems to be recommended at this time only under study conditions.

    Description / Table of Contents: Zusammenfassung Implantierbare subcutane Infusionskammern stellen eine attraktive Alternative zu konventionellen zentralvenösen Zugangssystemen dar. Bei 70 Patienten wurden 4 verschiedene Modelle implantiert. In 72% wurden bei 57 auswertbaren Patienten aufgrund einer Neoplasie bis zu 12 Cyclen einer Chemotherapie durchgeführt. Desweiteren erfolgten regelmäßige Blutentnahmen und teilweise parenterale Ernährung. Bei einer Gesamtimplantationszeit von 57 Jahren wurden 4970 Punktionen durchgeführt. Nach einer Gesamtbenutzungsdauer von 12,2 Jahren wurden 46 Komplikationen beobachtet, die in den meisten Fällen einfach therapiert werden konnten (temporärer Verschluß (12mal), Extravasion (14mal)). Neben Septumluxation (1mal) sowie Perforation (1mal) wurde ein Katheterbruch beobachtet. Ebenso machten 4 Implantattascheninfekte und 4 Katheter-bedingte Septicämien eine Explantation erforderlich. Infusionskammern stellten dennoch bei einer Komplikationsrate von 10 pro tausend Gebrauchstage für Arzt und Patient ein wenig störendes und zuverlässiges Zugangssystem mit deutlich besseren Ergebnissen dar, als nach Verwendung percutaner Katheter; Nachsorge und Betreuung bedürfen jedoch eines erfahrenen Teams.
    Notes: Summary Subcutaneously implanted infusion chambers represent a new method of central venous access. In 57 evaluable out of 70 patients, four different models of infusion chambers with an accumulative observation time of 57 years were implanted. In 72% of the patients, up to 12 cycles of polychemotherapy were administered. Parenteral nutrition and blood drawing were also performed. After 4,970 punctions of the system and 12.2 years of use 46 complications in 38 patients were observed, however, most were minor ones, such as temporary occlusions (12) and extravasations (14). Septum luxation (1), septum perforation (1), catheter fracture (1) and catheter migration (2) as well as 7 cases of septicemia or port-pocket-infection required explantation. Infusion chambers seem to be particularly suitable for intermittent and long-term chemotherapy and emergency bolus injections with a significant advantage (10 complications per one thousand days of use) compared to externally placed venous catheters. However, follow-up and care must be performed by a skilled team.

Neben Septumluxation 1lx) sowie Perforation (1x) wurde ein Katheterbruch beobachtet. Bei 4 Implantattaschendefekten traten nur bei 2 Patienten eine Katheter-bedingte Sepsis auf. Infusionskammern stellten für Arzt und Patient ein wenig störendes und zuverlässiges Zugangssystem dar, das jedoch einer sorgfältigen Nachsorge und Betreuung bedarf.

    Notes: Abstract  Background: Excellent results after resection of colorectal liver metastases are associated with a high rate of recurrence. Influenced by positive results of palliative and adjuvant treatment in advanced cancer, various chemotherapy regimens were evaluated to improve long-term results. Methods: The databases Medline and Cancerlit (1982–1998) gave information about 675 patients who were treated either by means of systemic, intra-arterial, intraportal or intraperitoneal administration before or after liver resection. Results: In general, the feasibility of an adjuvant treatment was tested. Proof has been furnished for the practicability of systemic and arterial therapy and for immunotherapy after liver resection whereas, for peritoneal and portal treatment, further studies are necessary. In a few non-randomised trials, it has been possible to discern a trend towards an improvement due to adjuvant postoperative therapy using historical or matched-pair control groups. Until now, only one of five randomised studies has been published. Six months of postoperative adjuvant intra-arterial treatment using 5-fluorouracil (1000 mg/m2 for 5 days every 28 days) and folinic acid (200 mg/m2 for 5 days every 28 days) was compared with observation only. Neither in the intention-to-treat nor in the as-treated analysis was median survival time (34.5 months versus 40.8 months and 39.7 months versus 44.8 months, respectively) significantly increased. As neoadjuvant treatment was successful in primary non-resectable patients, this approach is now being tested in resectable patients. Conclusion: Despite several theoretical reasons for post- or preoperative treatment in resectable patients, every approach should be tested using of controlled studies.
