    Notes: Abstract The evolution of the pattern of nucleolar organiser regions (AgNORs) in circulating lymphocytes during the stable phase and after chemotherapy in CLL was analysed. Peripheral blood smears were stained by the AgNOR technique at diagnosis, during observation follow-up in stable phase, or at the beginning and at the end of chemotherapy in patients with progressive disease. The changes in the AgNOR pattern were compared with those of TTM used as a tumour burden parameter. Among 52 cases that entered the study, 29 were in stable phase and 23 had progressive disease and received chemotherapy. During stable phase, the AgNORs as well as TTM remained constant. In treated patients, the relative reduction of tumour mass was correlated with a decrease in the percentage of lymphocytes containing one AgNOR cluster. The percentage of cells with one compact nucleolus before chemotherapy was inversely correlated with the relative amount of tumor reduction after treatment. We conclude that the AgNOR pattern in CLL describes the cell kinetic changes during the evolution of the disease and is a prognostic factor for tumor reduction after treatment.

    Description / Table of Contents: Zusammenfassung In einer prospektiv randomisierten Studie wurden 71 Leichennierenempfänger mit einer hohen, 73 mit einer niedrigen Cyclosporin-Dosis behandelt. Der mittlere Vollblutspiegel betrug 250 bzw. 500 ng/ml. Wegen unzureichender Funktion durchgeführte Biopsien zeigten bei ver gleichbarer Abstossungsrate in der niedrigen Gruppe in 46%, in der hohen jedoch in 70% eine Fibrose sowie hier zusätzlich Tubulusläsionen. Die 2-Jahres-Funktionsrate war in der niedrigen Gruppe geringfügig, aber nicht signifikant besser (82/77%). Aufgrund der dosisabhängigen histologischen Veränderungen sollte Cyclosporin demnach niedrig dosiert werden.
    Notes: Summary In a prospective random trial, 71 recipients of cadaver kidneys were treated with a high dose of cyclosporin A and 73 recipients with a low dose. The mean whole blood levels were 250 and 500 ng/ml, respectively. Biopsies performed for insufficient function showed fibrosis in 46% of the low-dose group compared to 70% in the high-dose group. Moreover, in the latter, tubular lesions were found. The rejection rates were similar in both groups. The 2-year function rate in the low-dose group was slightly but not significantly better (82/77%). Due to the dose-dependent histological findings, cyclosporin A should be given in low dosages.
