Zusammenfassung In der Behandlung der Choledocholithiasis konkurrieren Chirurgie und Endoskopie. Eine Eindämmung der Indikation zur Endoskopie erscheint vor allem beim Jüngeren notwendig. Eine retrospektive Analyse von 1979–1983 zeigt, daß die Operation lebensbedrohlicher endoskopischer Komplikationen (n=25) mit einer Letalität von 28% behaftet war (Literatur bis 50%). Der Altersdurchschnitt dieser Patienten lag mit 63 Jahren unter dem aller endoskopisch Behandelter. Die Letalität der endoskopischen Versager (n=26), die operiert werden mußten, war hingegen 0%. Die Letalität aller primär chirurgisch Behandelten (Durchschnitt 66 Jahre) betrug 4% und betraf überwiegend nur das hohe Alter. Die Therapie der Wahl muß daher die Chirurgie bleiben.

Summary Surgery and endoscopy are in competition in the treatment of choledocholithiasis. A reduction of endoscopic means, especially in younger patients, seems necessary. A retrospective analysis from 1979–1983 shows that surgery after life-threatening endoscopic complications (n=25) had a mortality of 28% (up to 50% in the literature). Mean age (63) of these patients was lower than that of all endoscopically treated patients. In contrast, surgical mortality of all endoscopic failures (n=26) was 0%. The mortality of primary surgery (mean age 66) was 4% and is due mainly to the higher age. Surgery remains the therapy of choice.
