    Description / Table of Contents: Abstract Background. In chronic pain patient's diagnostic and therapy planning many complex information must be evaluated and analysed. The variety of data cannot be mastered without assistance of a database tool adapted to these special need. Database environment. On the base of FileMaker Pro 5.0, a user-friendly and analysis-oriented database environment for documentation and quality assurance in pain therapy (QUAST) has been developed. QUAST has been tested for 2 years by 23 pain clinics in Germany including data of more than 14.000 patients. Among patients'demographic and clinical data all 335 fields of the validated German pain questionnaire are included in QUAST together with psychometric test procedures (CES, SES, PDI, SF-36). We developed a outcome questionnaire for the evaluation of treatment – related changes of outcome variables (103 fields), a daily questionnaire (11 fields) and a protocol to assess the effects of neural and other blockades (144 fields). All data of therapy and outcome can be documented (total number of the fields: 2661). The test results (e. g. in the form of total or compound scores) can be analysed and printed. For pain related diagnoses, procedures, blockades, drugs, expected and unexpected events hierarchical coding systems (altogether 72,157 data records) have been developed. These can be assigned automatically to coding systems like ICD-10 or ICP-M. Conclusion. Due to its ability of export into other databases and already enclosed routines QUAST permits an internal assurance of quality of process and outcome in pain management. It also can be an instrument of an external quality assurance. QUAST is at present the only computer system available in Germany, to fulfil the required prerequisites for adequate quality control.
    Notes: Zusammenfassung Hintergrund. In der Diagnostik und zur Therapieplanung bei chronischen Schmerzen müssen verschiedenste, komplexe Informationen erhoben und ausgewertet werden. Diese Datenvielfalt ist ohne Hilfe einer auf diese spezielle Problematik zugeschnittenen Datenbank nicht zu bewältigen. EDV-System. Auf der Basis von FileMaker Pro 5.0 wurde ein anwenderfreundliches und auswertungsorientiertes EDV-System zur Dokumentation und Qualitätssicherung in der Schmerztherapie (QUAST) entwickelt, das nach über 2-jähriger Erprobungsphase bereits in 23 schmerztherapeutischen Einrichtungen mit Einschluss von über 14.000 Patienten in Deutschland benutzt wird. Es erlaubt die Stammdaten- und Terminverwaltung und Arztbriefgenerierung. In QUAST können u. a. alle 335 Felder des validierten DGSS-Fragebogens, unter Einschluss der psychometrischen Testverfahren (ADS, SES, PDI, SF-36), ein hiermit kompatibler Verlaufsfragebogen (103 Felder), neu entwickelte Tagesfragebögen und Blockadeprotokolle (144 Felder) sowie eine Vielzahl weiterer Daten zur Therapie eingegeben werden (Gesamtzahl der Felder 2661). Die Testergebnisse (z. B. in Form von Summenscores wie beim ADS) werden ausgewertet und ausdruckbar graphisch dargestellt. Für die in der Schmerztherapie üblichen Diagnosen (MASK, IHS), Prozeduren, Blockaden, Medikamente, erwünschte und unerwünschte Ereignisse wurden hierarchische Kodierungssysteme (insgesamt 72.157 Wahlmöglichkeiten) entwickelt, die den üblichen, ebenfalls in QUAST implementierten Systemen wie ICD-10, ICPM zugeordnet werden können. Schlussfolgerungen. Aufgrund seiner Exportfähigkeit und bereits eingeschlossener Routinen erlaubt QUAST eine interne Prüfung der Prozess- und Ergebnisqualität schmerztherapeutischer Einrichtungen und kann als Instrument einer institutsübergreifenden externen Qualitätssicherung eingesetzt werden. QUAST ist zurzeit das einzige in Deutschland verfügbare EDV-System, das die hierfür geforderten Voraussetzungen erfüllt.

    Notes: The stabilities of Zn complexes of cysteine and histidine have been determined together with those with those of three derivatives of each n which one of their three donor functions (carboxyl, amino, and mercapto and imidazole, respectively) has been blocked. Using potentionmetric titrations of aqueous solutions, the 1:1 and 1:2 complexes of all for cysteine- and all four histidine-derived ligands are observed among te various species present (ligands, 1:1 and 1:2 complexes, and protonatd derivatives thereof). All cysteine-derived complexes are more stable than the corresponding histidine-derived complexes by 1-2 orders of magnitude for the 1:1 composition and by 1-6 order of magnitude for the 1:2 composition. For the cysteine series, the sequence of stabilities is cysteine 〉 cysteine ethyl ester ≫ N(α)-acetylcysteine ≫ SMethylcysteine. For the histidine series, the corresponding sequence is histidine 〉 histidine methyl ester 〉 N,N (imidazole)-dimethylhistidine 〉 N(α)-acetylhistidine. The order of stabilities can be explained by the relative strengths of the Zn-S vs. Zn-N coordination, y charge effects, and by chelate ring sizes.

    Notes: The dipeptides Ac-Cys-Val-OH (1a) and Ac-His-Val-OH (1b), H-Gly-Cys-OEt (2a) and H-Val-His-OEt (2b) as well as Z-Asp-Cys-OH (3a) and Z-Asp-His-OH (3b) (Z = benzyloxycarbonyl) were prepared, and their zinc complexation was investigated by potentiometric methods. They have in common that in addition to the cysteine thiolate or the histidine imidazole the second amino acid offers one donor function. The complex stabilities are very close to those of the corresponding difunctional derivatives of the single amino acids cysteine or histidine. This indicates the presence of seven- to ten-membered chelate rings.

    Notes: The eight possible types of dipeptides made up from histidine and cysteine, namely the unprotected ones H-His-Cys- OH (1) and H-Cys-His-OH (2), the N-protected ones Ac-His-Cys-OH (3) and Ac-Cys-His-OH (4), the C-protected ones H-His-Cys-OEt (5) and H-Cys-His-NH2 (6), and the fully protected ones Ac-His-Cys-OEt (7) and Ac-Cys-His-OEt (8) were prepared analytically pure. Their acid-base behavior and zinc complexation in solution were studied potentiometrically. In all cases 1:1 (zinc:peptide) complexes are the dominant species near neutral pH. 1:2 complexes could only be detected for 4, 6, 7, and 8, and precipitations occurred in basic media. From equimolar mixtures of the peptides and zinc salts (chloride, iodide, perchlorate) solid complexes were obtained upon addition of a base. They always contain coordinating anions (chloride, iodide, or trifluoroacetate which was introduced with the peptide). They have the composition LZnX for L = 5-8 and L4Zn5X2 for L = 1-4. It is proposed that all complex species observed contain tetrahedral zinc, that they are monomers in dilute solution and thiolate-bridged oligomers in the solid state, and that the peptides as a rule occupy at least three coordination sites.
