    Notes: Abstract The smooth muscle relaxation induced by nitroglycerin is hypothesized to be mediated by an increase in the cytoplasmic concentration of guanosine 3′,5′-monophosphate (cGMP) and subsequent dephosphorylation of the 20-kilodalton myosin light chain (MLC). We investigated this hypothesis in procine coronary arterial smooth muscle stimulated with histamine (3 μM) or K+ (30 mM). Stimulation of [32P]Pi-labeled muscle with histamine or K+ for 2 min resulted in a four- or 6.2-fold increase, respectively, in the incorporation of32P into MLC. After 48 min of exposure to histamine. MLC phosphorylation decreased to the basal level and the phosphorylation of desmin, synemin, and of three unidentified cytosolic proteins was increased. K+ stimulation resulted in a sustained increase of MLC phosphorylation but had no effect on the phosphorylation of desmin, synemin, or the three unidentified cytosolic proteins. Application of nitroglycerin (1 μM) 48 min after histamine stimulation inhibited the phosphorylation of desmin, synemin, and the three cytosolic proteins. The sustained phase of histamine-induced contraction was also inhibited to a greater extent then the acute phase of histamine-induced contraction and both the acute and sustained phases of K+-induced contraction. These results suggest that MLC phosphorylation is required for both phases of K+-induced contraction, whereas phosphorylation of intermediate filament proteins is required for the sustained phase of histamine-induced contraction. Intermediate filament proteins, rather than MLC, may also be the target for the relaxant action of nitroglycerin during histamine-induced sustained contraction.

    Notes: Abstract Surgical salvage for stomal recurrence is a formidable problem for head and neck surgeons. The two factors of considerable significance are resectability and establishment of a safe anterior mediastinal tracheostoma. A case of stomal recurrence invading the cervicothoracic esophagus and upper mediastinum is presented. Total esophagectomy and upper mediastinal dissection was performed. The esophagus was reconstructed immediately with a pedicled gastric flap. The omentum on the gastric pedicle was wrapped around the trachea to reduce the likelihood of erosion into the great vessels and to supplement the lateral blood supply to the trachea. No serious postoperative complications were observed. We believe that the total esophagectomy improved the resectability, and that the bulk of the gastric pedicle and the use of the omentum prevented significant postoperative complications associated with an anterior mediastinal tracheostoma.

    Notes: Abstract A body surface potential mapping system equipped with a microprocessor has been developed. It enables high-speed real-time and simultaneous observation of e.c.g. data of 96 lead points in a character display mode. The machine has functions not only to perform basic data processing such as baseline correction and linear interpolation for malfunctioned electrodes but also to generate an isopotential contour map, record data onto a floppy disc, make hard copies of displayed patterns and characters, etc. To increase the operation speed, all the assembler programs were put in one module. Thus, an operator can easily recognise the spatial motion pattern of the body surface potential map and, in addition, find malfunctioned channels by observing the pattern. A belt type electrode system allows quick and adaptable attachment. The practical usefulness of the system has been proved with the co-operation of cardiologists with over 120 cases of cardiac disease.
