    Notes: Abstract The purpose of this study was to assess the effects of spontaneous and instrumented deliveries, the baby's birthweight, the presence of stress incontinence and the woman's age and weight on bladder neck (BN) position and mobility using perineosonography, a simple non-invasive method, and to compare these results with those from continent nulliparous controls. Two hundred and fourteen women, including 74 nullipara, 29 para-1, 64 para-2 and 3, 16 with previous forceps deliveries and 32 with stress incontinence, underwent perineosonography with measurements of BN position and backwards/downwards displacement of BN using a two-axis calculation system. Results showed that bladder neck position undergoes a significantly backwards and downwards displacement on assuming an upright position. When compared with nulliparous controls, the bladder neck position at rest was the same in all groups of parous women in the supine/standing positions, but lower in patients having undergone forceps delivery (standing position) and in stress incontinent patients (both supine and standing); the bladder neck position during Valsalva was significantly lower in all groups of patients in the standing position, but the only significant difference in the supine position was seen in stress incontinent patients; the extent of bladder neck displacement was not significantly different between the groups, except in stress incontinent patients. A strong correlation (r=0.66) was found between the relative importance of backwards and downwards displacement, but no correlation was found between bladder neck displacement and baby's birthweight or patient's weight. It was concluded that compared to nulliparous continent patients, normal and instrumented delivery induces no modifications of BN position at rest, but is responsible for a lower bladder neck position during Valsalva in the standing position only. The extent of BN displacement is virtually the same in all groups of patients in both positions, except for stress incontinent patients, who have a significantly lower and more mobile bladder neck, the values of which overlap to a great extent (sensitivity of 78%/75% for a displacement 〉14 mm) within those seen in continent nulliparous women. The correlation between the importance of backwards and downwards displacement is strong, making superfluous a two-dimensional coordinate system for routine BN position/mobility assessment.

    Notes: Abstract The aim of the study was to assess and compare urethral sphincter (US) function and bladder neck (BN) behavior in pregnant and non-pregnant women. Urethral pressure profile parameters, intravaginal-anal pressures and BN position/mobility were compared in both pregnant and non-pregnant women. It was found that pregnancy results in a decreased maximal urethral closure pressure (MUCP)/area of continence at rest and at stress, decreased pressure-transmission ratio (PTR) values and backwards displacement of the bladder neck. No significant changes in intravaginal-anal pressures and BN mobility/downwards displacement were seen. Compared with stress continent pregnant patients, pregnant patients with stress urinary incontinence (SUI) have a lower MUCP at rest/stress and area of continence at stress, but show no difference in BN behavior. Regression analysis shows no correlation between advancing pregnancy and any of these parameters. It was concluded that pregnancy induces diminished US function and backwards displacement of the BN. Compared to continent pregnant patients, pregnant women with SUI also have a diminished US function. US parameters are not significantly modified by advancing pregnancy.

    Description / Table of Contents: Summary Purpose: Providing medical reports on wards and ambulatory settings in electronic form can improve the quality of health care delivery. Radiology was chosen as an example to demonstrate how to implement uniform access to clinical data in a hospital. Method: Medical professionals at the university hospital in Munich, Großhadern, have access to all radiological reports of their patients. Web browsers are used as a front end. A centralized administration of users and patients is in place. Results: Centralized access control and patient selection guarantee uniform access to all applicable data via intranet and controls access rights. The high acceptance of this service is reflected by the high number of 150 requests per day. Discussion: Access to radiological reports within the Grosshadern University Clinics was enabled via web browsers in a short time. The high acceptance of the system also proves its easy use. Integration of the system in a centralized user and patient identification system supports the unified access to clinical data.
    Notes: Zusammenfassung Fragestellung: Durch die elektronische Bereitstellung von klinischen Befunden auf Stationen und in Ambulanzen kann die Versorgungsqualität verbessert werden. Am Beispiel der Radiologie soll gezeigt werden, wie ein einheitlicher Zugriff auf klinische Daten in einem Krankenhaus realisiert werden kann. Methode:Über Intranet wird den Ärzten und dem Pflegepersonal im Klinikum Großhadern der Zugriff auf alle radiologischen Voruntersuchungen ihrer Patienten ermöglicht. Web-Browser werden dabei als Benutzerinterface benutzt. Eine zentrale Benutzer- und Patientenverwaltung wird eingesetzt. Resultate: Die zentrale Zugangskontrolle und Patientenauswahl sorgen für einen einheitlichen Zugriff auf alle verfügbare Daten über das Intranet und regelt die Zugriffsrechte. Der Dienst wird mit durchschnittlich 150 Abfragen pro Tag im Klinikum gut angenommen. Diskussion: Durch den Einsatz der Internet-Technologie konnte die Bereitstellung radiologischer Befunde im gesamten Klinikum Großhadern ohne großen Aufwand realisiert werden. Die große Akzeptanz des Systems zeigt gleichzeitig die einfache Bedienbarkeit durch die Ärzte und die Pflegekräfte. Die Einbindungen in eine zentrale Benutzer- und Patientenidentifikation erlaubt einen einheitlichen Zugriff auf klinische Daten.

    Description / Table of Contents: Summary Problem: The goal of the current article is to demonstrate how qualitative and monetary effects resulting from an integrated RIS/PACS installation can be evaluated. Methods: First of all, the system concept of a RIS/PACS solution for a university hospital is defined and described. Based on this example, a generic method for the evaluation of qualitative and monetary effects as well as associated risks is depicted and demonstrated. To this end, qualitative analyses, investment calculations and risk analysis are employed. Results: The sample analysis of a RIS/PACS solution specially designed for a university hospital demonstrates positive qualitative and monetary effects of the system. Under ideal conditions the payoff time of the investments is reached after 4 years of an assumed 8 years effective life of the system. Furthermore, under conservative assumptions, the risk analysis shows a probability of 0% for realising a negative net present value at the end of the payoff time period. Conclusion: It should be pointed out that the positive result of this sample analysis will not necessarily apply to other clinics or hospitals. However, the same methods may be used for the individual evaluation of the qualitative and monetary effects of a RIS/PACS installation in any clinic.
    Notes: Zusammenfassung Fragestellung: Die vorliegende Arbeit soll Möglichkeiten aufzeigen, wie qualitative und monetäre Effekte, die durch die Installation einer integrierten RIS-PACS-Lösung entstehen, bewertet werden können. Methode: Dazu wird zunächst ein RIS-PACS-Systemkonzept für eine Universitätsklinik definiert und beschrieben. Auf diesem Systemkonzept aufbauend wird eine allgemeingültige Methode zur Bewertung qualitativer und monetärer Effekte und Risiken beschrieben und an einem Beispiel erläutert. Hierzu werden Methoden der Nutzwertanalyse, Investitionsrechnung und Risikoanalyse herangezogen. Ergebnisse: Die beispielhafte Analyse einer auf eine Universitätsklinik zugeschnittenen RIS/PACS-Lösung zeigt positive qualitative und monetäre Effekte des Systems. Unter idealen Bedingungen findet die Amortisation der Investition nach den ersten 4 Jahren der angenommenen 8jährigen Nutzungsdauer des Systems statt. Die Risikoanalyse zeigt ferner unter konservativen Annahmen eine Wahrscheinlichkeit von 0% für die Realisierung eines negativen Kapitalwerts nach Ende der Nutzungsdauer. Schlußfolgerungen: Es bleibt festzustellen, daß das positive Ergebnis dieser beispielhaften Analyse nicht zwangsläufig auch für andere Kliniken eintreten muß und somit nicht übertragbar ist. Die dargestellte Methode ermöglicht allerdings eine individuelle Ermittlung von qualitativen und monetären Effekten einer RIS/PACS-Lösung für jede Klinik.
