Abstract Painful scoliosis may result from persistent muscle spasm caused by an osseous lesion located on one side of a vertebra or the posterior end of a rib. The scoliosis is always convex to the opposite side and the abnormality is situated invariably in the concavity of the curve. In this series of 15 cases the most common cause was the osteoid osteoma-benign osteoblastoma complex. Other causes included pyogenic infection, eosinophil granuloma and aneurysmal bone cyst. Clinical failure to appreciate the cause of a painful scoliosis and difficulty in detecting early radiological signs may delay diagnosis by many months.
