    Notes: Abstract In a retrospective study 108 patients (average age: 56.5 years) could be evaluated, who were treated with a fascial sling plasty because of pure genuine stress incontinence (n=90) or combined stress/urge incontinence (n=18). 72 patients (66.6%) had recurrent incontinence after previous surgery. The average follow-up was 5 years. Patients without previous surgery related to urinary incontinence (n=36) showed the best results and remained continent in 78% (n=28). Patients with 1 to 4 (average 1.4) previous surgical procedures because of incontinence (n=52) remained continent in 62% (n=32), and in those who had between 1 and 6 (average 3.6) previous surgical procedures using even alloplastic material the continence rate dropped to 60% (n=12). In patients with pure genuine stress incontinence the overall success rate was 74.7%, whereas in cases with a proven preoperative urge component, the success rate dropped to 37%. Fascial sling plasty is a very useful procedure to treat patients with primary and especially recurrent urinary incontinence, when a functional urethra is missing. However, one has to be very cautious with patients in whom a preoperative urodynamic evaluation shows detrusor instability.

    Description / Table of Contents: Zusammenfassung Bei Patienten mit fortgeschrittenem Prostatakarzinom ist die suffiziente Therapie schmerzhafter Knochenmetastasen primäres, interdisziplinäres Behandlungsziel. Vorrangig erscheint die Erhaltung der Lebensqualität, nicht die Lebensverlängerung. Neben einer oralen Schmerzmedikation und lokalen Schmerzbestrahlung bieten systemische Radionuklidtherapien hilfreiche, risikoarme nuklearmedizinische Therapiealternativen in der Palliation schmerzhafter, multilokulärer osteoplastischer Knochenmetastasen. In Abhängigkeit vom gewählten Radiopharmazeutikum werden Ansprechraten und Schmerzreduktion in 65–80 % beschrieben. Die Dauer der Schmerzlinderung liegt im Mittel bei 6–12 Wochen. Während dieser Zeit kann die nebenwirkungsreiche, morphinhaltige Schmerzmedikation deutlich reduziert und in einigen Fällen ganz abgesetzt werden, was wiederum die Lebensqualität verbessert. Nach Abklingen der geringgradigen Myelodepression kann die Radionuklidtherapie wiederholt werden. Nachteilig bleibt allein der zweitägige stationäre Aufenthalt aufgrund geltender Strahlenschutzverordnung.
    Notes: Summary For patients with advanced prostate cancer efficient therapy of painfull bony lesions is the primary goal of interdisciplinary treatment strategies. Preservation of quality of life appears to be the main aim rather than prolongation of life. Apart from oral pain relief and local irradiation systemic treatment with radionuclides offers low-risk radiotherapeutic strategies for the palliation of painful, multifocal osteoplastic bone metastases. Depending on the radiopharmaceutical substance chosen response and reduction of pain are described in 65–80 %. The duration of pain relief lasts between 6–12 weeks. During this time the morphine based medication can be reduced and in some cases withdrawn which positively effects quality of life. After improvement of myelosuppression treatment with radionuclides can be repeated. Patients have to be hospitalized for two days because of protection from radiation procedures.

    Notes: Abstract Because of their resistance to fragmentation, treatment of brushite stones is a big problem. This study was performed to look for an improvement in therapeutic strategies by using artificial stones made of brushite (Bon(n)-stones), which are comparable to their natural counterparts. Using an ultrasound transmission technique, longitudinal wave propagation speeds were measured at different time intervals during treatment with hemiacidrin. From these and density measurements, transverse wave speed, wave impedance and dynamic mechanical properties of the artificial stones were calculated. Moreover, the microhardness of artificial stones was measured and investigations on shock wave lithotripsy (SWL) combined with initial chemolytic treatment of the stones were performed. The suggestion was confirmed that stone fragility and thus SWL can be improved by varying the physical properties of brushite stones through treatment with hemiacidrin. Additionally, we demonstrated the efficacy of Suby G in dissolving artificial brushite stones using an experimental arrangement simulating the physiological conditions in the upper urinary tract. Moreover, the efficacy of four different intracorporeal lithotripsy devices (electrohydraulic, pneumatic, laser and ultrasound) was tested and it was shown that electrohydraulic lithotripsy seems to be the best system for comminution of brushite stones.
