ZusammenfaBung Die Symphyse und die Sacro-Iliacal-Gelenke müBen aus mechanischen überlegungen heraus als Gelenke angesehen werden. Dabei wird die Symphyse in den beiden senkrecht zueinander stehenden Ringebenen vorwiegend auf Zug und Schub beansprucht. Das Anbringen einer Zuggurtungsplatte gleichzeitig an beide Schambeinäste führt aus mechanischer Ursache zur Lockerung der Schrauben. Es wurde experimentell ein neues Operationsverfahren untersucht. Es werden 2 L-Platten an die oberen Schambeinäste angebracht. In die freien Löcher der kurzen Schenkel der Platten wird eine spezielle Klemme eingehängt, die durch Nachspannen eine KompreBion von 30 kp erzeugt. Die Klemme selbst toleriert Zugbeanspruchungen von über 120 kp. Bereits bei einer Zugbelastung von ca. 50 kp kommt es zu einem Nachgeben der spongiösen Struktur der Schambeinäste mit Lockerung der Schrauben. Die Indikation zur Anwendung dieses Verfahrens besteht bei frischen Harnröhrenrupturen, bei statisch relevanten Verschiebungen bei vorderen Bekkenringfrakturen, zur Stabilisierung des vorderen Beckenringes nach Korrektur von Harnröhrenstrikturen und in Einzelfällen bei alten Menschen.

Summary The pubic symphysis and the sacroiliac joint must be regarded as joints on mechanical grounds. The pubic symphysis is subject mainly to push and pull streB in the two ring planes that are perpendicular to each other. For mechanical reasons, fixing an AO bone plate simultaneously to both leads to loosening of the screw. A new operative technique was studied experimentally. Two L plates were attached to the upper branches of the pubic bone. A special clamp, which produces a compreBion of 30 kp by being subsequently tightened, was set into the free holes in the short arms of the plates. The clamp itself has a streB tolerance of over 120 kp. Even a tensile streB of 50 kp results in the slackening of the structure of the spongy pubic bone and in the loosening of the screw. This procedure can be applied in cases of fresh urethra ruptures, staticly relevant displacements in fractures of the anterior pelvic ring after correction of urethral strictures, and in individual cases in old people.
