Zusammenfassung Nach erfolgloser medikamentöser Therapie einer Detrusorhyperreflexie kann Kontinenz und Druckstabilität nur durch eine operative Intervention erreicht werden. Hierzu stehen mehrere Verfahren zur Verfügung. Neben der Ileumaugmentation als Standardmethode hat sich nach Inauguration durch Cartwright und Snow, sowie Stöhrer et al. die Autoaugmentation der Harnblase etablieren können. Die Autoren berichten hier über ihre Erfahrungen mit der Detrusormyektomie als operativ einfachem und standardisiertem Verfahren. Von 1989 bis 1998 wurden 62 Patienten nach dieser Methode operiert. Die Indikation, die Operationstechnik und die Ergebnisse werden dargestellt.

Summary After unsuccessful medical treatment of a neurogenic hyperreflexive bladder dysfunction further treatment options are the ileal bladder augmentation, the detrusor-myectomy (bladder auto-augmentation) and the sacral root deafferentation. All techniques lead to a low-pressure bladder with enlarged storage volume. The authors here report about their personal experience with the detrusor myectomy in 62 patients with hyperreflexive neurogenic bladder dysfunction from 1989 until 1998. Indication, surgical technique, and long-time results are presented.
