Zusammenfassung Es wird über einen Fall einer posttraumatischen Hämobilie aus dem linken Leberlappen berichtet. Infolge einer atypischen arteriellen Gefäßversorgung kam es sowohl nach der Ligatur der A. hepatica sin. als auch nach einer zusätzlichen vollständigen Dearterisierung zu Rezidivblutungen. Als Ursache konnte durch transumbilicale Portografie ein Defekt im portalen System des linken Leberlappens verantwortlich gemacht werden. Schließlich wurde eine Lebersegmentresektion erforderlich. Der Fall zeigt, daß eine selektive Arterienligatur zur Behandlung der Hämobilie nur dann erfogreich ist, wenn sofort eine vollständige Dearterialisierung erreicht wird. Hämobilie kann aber zusätzlich durch eine portobiliäre Verbindung ausgelöst werden - unter Umständen ist eine entsprechende Leberresektion notwendig.

Summary A case of traumatic hemobilia is reported. After ligation of the left hepatic artery and additional dearterialisation a third bleeding period occured due to a porto-hepatic communication, diagnosed by a transumbilical portography. A left hepatic lobectomy was finally necessary to achieve hemostasis. This case demonstrates that a selective artery ligation for the treatment of hemobilia is only successful if a complete dearterialisation is performed at the first operation. Furthermore a porto-hepatic communication may require partial hepatectomy.
