Notes: Summary We performed 96 operations on 57 patients with Crohn's disease between 1972–1979. The operative mortality was 7 %. Absolute indications for an emergency operation are massive bleeding, perforation, acute ileus, toxic megacolon and acute appendicitis. Primary resection of the involved intestinal segment is generally preferred. An appendectomy is performed in presence of Crohn's ileitis whether there is an appendicitis or not. The removal of terminal ileum and cecum is only necessary if ileocecal Crohn's disease is found. Indications for elective surgery are chronic fistulae, such as entero-enteric or perianal fistulae, urological complications as for instance ureteric stenosis or enterovesical fistula, chronic ileus and unsuccessful conservative treatment with progressive cachexia.
