Zusammenfassung Anhand von Verlaufsdaten 66 chirurgischer Patienten mit einer bakteriologisch gesicherten Peritonitis sollten „harte” und „weiche” Kriterien zur Beurteilung des Schweregrades differenziert werden. Untersuchungskriterien: 1) regionale Ausbreitung, 2) Entwicklung eines Organod. Systemversagens, 3) Leukocytose/Thrombopenie, Körpertemperatur, 4) die Plasmamediatoren Endotoxin/Prostaglandin F2α (PGF2α), 5) Anamnesedauer (Erstsymptom bis OP), 6) Anzahl erforderlicher Reoperationen und 7) Lebensalter. Zuzuordnende Schweregrade: Grad 1: überlebt, Grad 2: mit Komplikationen überlebt, Grad 3: nicht überlebt. Danach erweisen sich als „harte” Kriterien: 1) Lokalisation, 2) Anamnesedauer, 3) Leukocytose, 4) Endotoxin 〉 100 Eu/ml, PGF2α 〉 500 pg/ml, 5) Körpertemperatur 38,5°C.

Summary “Strong” and “weak” criteria for the classification of the severity of disease were analyzed in 66 patients with bacterial peritonitis. Discriminations were made on the basis of: (1) regional spread of peritonitis; (2) organ and/or system failure; (3) leukocytosis, thrombocytopenia or abnormalities of body temperature; (4) levels of the plasma mediators endotoxin and prostaglandin F2α, (PGF2α); (5) duration of illness prior to surgery; (6) number of further operations necessary; (7) age. The corresponding degree of severity was classified as follows: patients with grade 1 survived without complications, those with grade 2 survived with organ complications, and grade 3 was assigned to nonsurvivors. Criteria that appeared to be “strong” were: (1) localization; (2) duration of history; (3) leukocytosis; (4) endotoxin level 〉 100 Eu/ml, and level of PGF2α 〉 500 pg/ml; (5) body temperature 〉 38.5°C.
