Zusammenfassung Osteosynthesematerial ist bei instabilen pertrochanteren Frakturen greiser Patienten unter Sofortbelastung hohen varisierenden Biegebeanspruchungen ausgesetzt. Mechanisches Versagen der Osteosynthese ist daher häufig (in einem Drittel der Fälle unseres Krankengutes). Die Fehleranalyse zeigt, daß schädliche Biegekräfte nur durch statisch-biomechanisch sinnvolle Reposition mit überkorrigierender Valgisation und Medialisation gemindert werden. Anatomisch exakte Reposition und stabile Fixation allein garantieren keine Sofortbelastbarkeit.

Summary In case of unstable intertrochanteric femoral fractures of elderly men osteosynthetic material is exposed to high bending forces during locomotion. Mechanical failure is quite frequent. The analysis of our ] cases with failure of internal fixation revealed that harmful bending forces are dimished only by suitable reduction with valgus fixation and medial displacement according to static-biomechanical principles. Immediate stability however is not achieved by internal fixation of the fragments in an anatomical position.
