    Notes: Summary Serum elastase 1 was determined in the serum of 38 patients with acute pancreatitis, using specific radioimmunoassay technique. Serving as controls were 36 healthy people, 33 patients with chronic pancreatitis, 49 patients with various GI-tract diseases, and 6 patients with pancreatic carcinoma. Sensitivity of elastase 1 for the diagnosis “acute pancreatitis” was 97% after admission to the hospital and 100% within 48 h after onset of acute pancreatitis. The determination of elastase 1 is clearly superior to that of trypsin, pancreatic lipase, or pancreatic amylase, if diagnosis has to be made more than 48 h after the onset of the disease. The specificity is restricted, because there are some cases with chronic pancreatitis and GI-diseases with raised values. There is no possibility to estimate the severity of acute pancreatitis by measuring serum elastase 1.

    Description / Table of Contents: Zusammenfassung 61 Patienten mit akuter Pankreatitis wurden nach morphologischen Kriterien eingeteilt in zwei Gruppen: 31 Patienten mit interstitiell ödematöser und 30 Patienten mit nekrotisierender Pankreatitis. über 14 Tage wurden im Serum alpha-l-antitrypsin, alpha-2-makroglobulin, Komplement C3, Komplement C4 und das CRP bestimmt. Die Mittelwerte aller fünf gemessenen Parameter unterscheiden signifikant die beiden Pankreatitis-Kollektive. Die Treffsicherheit zum Nachweis der Pankreasnekrose betrug 82 % für das alpha-2-makroglobulin und 95 % für das CRP. Hierin bieten sich neue Wege zum Staging der akuten Pankreatitis an.
    Notes: Summary Sixty-one patients with acute pancreatitis were divided into two groups following morphological evaluation: 31 patients with interstitial edematous pancreatitis and 30 with necrotizing pancreatitis. For 14 days the serum was analyzed for a-l-protease inhibitor, a-2-macroglobulin, complement C3 and C4 and for C-reactive protein (CRP). The means of all of the five parameters measured were significantly different between the two groups of pancreatitis patients. Alpha-2-macroglobulin showed a 82 % necrosis detection rate and CRP 95 %. These two factors offer a new dimension in staging acute pancreatitis.

    Notes: Abstract Background: During the last decade, many molecular alterations have been described for pancreatic carcinomas. However, the clinical and prognostic value of these alterations has been discussed and is controversial. Methods: An immunhistochemical study was performed in 82 cases of adenocarcinoma of the pancreas. Using specific antibodies, expression of EGF, EGF-receptor, cERB-B2, p53, p21CIP1, cyclin-D1, BCL-2, CD95 and KI67 was evaluated. Results: Overexpression of the different molecules was found in 44–69% of the pancreatic carcinomas. With regard to clinico-pathological features, p53 positivity was more frequently found in advanced and undifferentiated tumours (P〈0.05), EGF overexpression was significantly more frequent in advanced tumours (P〈0.05) and CD95 overexpression was observed to a greater extent in undifferentiated tumours (P〈0.05). Besides cyclin-D1, none of the molecules tested was of prognostic significance. Patients whose tumours expressed cyclin-D1 lived significantly shorter than patients with cyclin-D1-negative tumours. However, in subgroup analyses of patients with the same tumour stage or tumour grade, even cyclin-D1 expression had no prognostic significance. Conclusion: These results demonstrate that the prognostic significance of the molecules tested here is low. Nevertheless, with regard to tumorigenesis and tumour biology of pancreatic carcinoma, determination of molecular alterations could provide important information about pancreatic cancer.

    Description / Table of Contents: Zusammenfassung Durch regelmäßige, EDV-gesteuerte Nachsorge bei primär kurativ bzw. fraglich kurativ an einem colorectalen carcinomoperierten Patienten wurde bei über einem Drittel der Rezidivpatienten die Rezidivdiagnose vor Ausbildung klinischer Symptome gestellt. Durch den frühen Zeitpunkt der Diagnosestellung konnten doppelt so viele Patienten mit Nachsorge gegenüber Patienten ohne Nachsorge kurativ am Rezidiv operiert werden. Weiterhin wurde deutlich, daß eine regelmäßige Nachsorge eine Verlängerung der Überlebenszeit mit sich bringt.
    Notes: Summary In patients with colorectal carcinoma operated curatively or questionable curatively, the computer-controlled postoperative care showed that in more than one third of the patients with recurrence the diagnosis was made before clinical symptoms appeared. By early detection, the recurrence could be resected curatively in twice as many patients with regular postoperative care as in patients without regular care. Furthermore could be proved that regular postoperative care caused a clear prolongation of the survival time.
