Zusammenfassung Die Tibiaetagenfraktur ist durch einen mit 56% hohen Anteil offener Verletzungen und mit 11,5% relativ häufige schlechte Behandlungsergebnisse gekennzeichnet. Unsere Untersuchungsergebnisse an 68 Patienten zeigen, daß das Ausmaß der Weichteilschädigung den Heilungsverlauf erheblich beeinträchtigt. Trotz der problematischen Durchblutung der Tibia und der starken Gefährdung bei Etagenbrüchen ist die Marknagelung mit einem dünnen Nagel ein gutes Verfahren. Bei der Versorgung höhergradig offener Verletzungen sollte der Fixateur externe bevorzugt werden, da dieses Verfahren keine wesentliche zusätzliche Weichteilschädigung mit sich bringt.

Summary The segmental fracture of the tibia is characterized by a high percentage (56%) of open injuries and a large proportion (11.5%) of unsatisfactory results. Our investigations of 68 patients showed that healing is related to the degree of soft tissue involvement. Because of the problematic blood supply of the tibia and the major circulatory impairment in segmental fractures, fixation with a thin nail is a good technique. In open fractures with major soft-tissue injuries external fixation is preferable, as it causes no additional soft tissue injury.
