Zusammenfassung Bei 200 MS-Patienten wurden die HLA-A, -B, -C und -D bestimmt. 64 dieser Patienten hatten eine Liquoruntersuchung. Das Auftreten dieser Antigenen wurde mit dem Krankheitsverlauf korreliert und dabei wurden insbesondere wesentliche krankheitsbeeinflussende Faktoren wie Alter der Patienten bei Beginn der Erkrankung, klinische Verlaufsform (Typ) und intrathekale IgG-Synthese berücksichtigt. Keine Korrelation wurde gefunden zwischen HLA-Haplotypen und dem Tempo oder Typ des Krankheitsverlaufes. Patienten, die nach einem Alter von 31 Jahren erkrankt waren, zeigten sich öfter (P=0.016) als Träger des DR2-Determinants als Patienten mit einem früheren Krankheitsbeginn. Obwohl die meisten Patienten mit einem IgG-Index größer als 1.5 oder mit mehr als 15 oligoklonalen Bändern im Liquor DR2-Träger waren, wurde in der intrathekalen IgG-Synthese bei HLA-DR2-positiven und -negativen Patienten kein statistischer Unterschied gefunden.

Summary The HLA-A, -B, -C and -D were determined in 200 multiple sclerosis (MS) patients, 64 of whom underwent a CSF examination. Their frequencies were correlated with the rate of disease progression and with factors thought to influence disease progression, including onset age, type of clinical course and intrathecal IgG synthesis. No correlation was found between HLA haplotypes and progression rate or type of disease course. Patients starting MS after age 31 years carried the DR2 determinant more frequently (P=0.016) than patients with onset before this median age. Although the greatest proportion of patients with an IgG index greater than 1.5 or more than 15 oligoclonal bands in their CSF were DR2 carriers, no statistical difference was found in the intrathecal IgG synthesis of HLA-DR2 (+) and (−) patients.
