    Notes: Abstract. Nitrate leaching and pasture (Lolium perenne / Trifolium repens) yields were measured on monolith lysimeters (80 cm diam. × 120 cm depth) of a Templeton sandy loam soil (Udic Ustochrept), following repeated applications of dairy shed effluent (DSE) or ammonium fertilizer (NH4Cl), under spray (50 mm/month) or flood (100 mm/month) irrigation. Applications of DSE at 400 kg N/ha per annum resulted in significantly less nitrate leaching (8–25 kg N/ha per yr) compared with NH4Cl (28–48kg N/ha per yr) (P 〈 0.01). Over the two year period, the total mineral N (predominantly nitrate) leached was equivalent to 2.5–3.7% of the total N applied in the DSE and 8.7–9.8% of the N applied in the NH4Cl. There was a trend of slightly less nitrate leaching under the flood irrigation than under the spray irrigation, probably because of the greater potential for denitrification under the wetter conditions. Average nitrate concentrations in the leachate were generally below the drinking water standard except in the NH4Cl treatment under spray irrigation where it averaged 10 mg NO3-N/l over the two year period. DSE was equally as effective as NH4Cl in stimulating pasture dry matter production. Annual nitrogen uptakes were similar for the DSE (343 kg N/ha) and NH4Cl (332–344kg N/ha) treatments in the first year but were higher in the DSE (361–412 kg N/ha) than in the NH4Cl (324–340 kg N/ha) treatments in the second year. Pasture uptakes of phosphorus and sulphur were also higher in the DSE than in the NH4Cl treatments in the second year. The results emphasize the need to set different regulatory limits for land application of organic wastes of various types and for N fertilizers.

    Notes: We present a case of phenytoin-induced toxic epidermal necrolysis resulting in 60–70% skin involvement. Systemic corticosteroids and prophylactic antibiotics used initially were discontinued, and subsequent management concentrated on intensive supportive treatment. The patient survived, but is left with disabling ocular complications.

    Notes: Cellular neurothekeoma is a recently recognized benign cutaneous neoplasm, which is currently regarded as being of nerve sheath origin and is thought to represent a variant of conventional neurothekeoma (dermal nerve sheath myxoma). Nine new cases presenting predominantly in adolescents or young adults are described. Morphologically they were characterized by short fascicles or small nests of palely eosinophilic epithelioid or spindle-shaped cells which ramified in an ill-defined manner between dermal collagen bundles. Myxoid matrix was absent or sparse. Scattered normal mitoses and multinucleate giant cells were often present. Immunohistochemically all nine cases were strongly NK1/C3 positive, seven were weakly NSE positive and three were smooth muscle actin positive. Staining for S-100 protein, PGP 9.5, epithelial membrane antigen and desmin was negative in all cases. In view of its distinctive architecture and immunophenotype, both of which are totally different from conventional neurothekeoma, it is proposed that cellular ‘neurothekeoma’ is a separate discrete entity which may represent an epithelioid variant of pilar leiomyoma.

    Notes: Sections from 95 skin lesions excised at pigmented lesion clinics in England and Scotland were studied by eight histopathologists in order to evaluate consistency in the use of histopathological terms for features of diagnostic and prognostic importance for cutaneous malignant melanoma. The level of agreement (kappa) amongst the panel improved after discussion and re-definement of criteria for several features. These included, architectural and nuclear atypia, pagetoid infiltration and radial and vertical growth phases. A high level of agreement was achieved for an overall benign or malignant diagnosis (κ = 0.77) but use of more specific terms such as benign naevi with atypia and melanoma ≤0.76 mm thickness, was associated with only an intermediate level of agreement. Of the original diagnosis of melanoma, 17% were re-classified by the panel as benign with atypia and 2% reported to be benign were judged to be melanoma. This reflected the high proportion of borderline lesions in the study. The use of standardized diagnostic criteria with precise definitions has been shown to improve consistency in diagnosis and it is recommended for general application. From this should emanate more reliable incidence figures for thin melanoma, and improved understanding of the nature of these early lesions, to the benefit of patient and clinician alike. The poor concordance in distinguishing severe dysplasia in the junctional component of melanocyte proliferations from melanoma in situ and superficial dermal invasion improved only modestly despite intensive efforts. Since melanoma in situ and severe dysplasia cannot be distinguished by objective measurements and since their clinical management is the same, the panel suggests that attempts to separate them in diagnostic reports should be discontinued and they could both be referred to as melanocytic intraepidermal neoplasia (MIN). If it becomes accepted that dermal invasion without a vertical growth component can also be managed identically to MIN, then this invasive radial phase may be appropriately referred to as microinvasion and linked to MIN for the purposes of clinical management.
