    Notes: A panel of 28 monoclonal antibodies against Piscirickettsia salmonis was produced using a purified fraction of the bacterium. To determine their specificity to the pathogen, the antibodies were assayed by ELISA and indirect immunofluorescence microscopy. Six monoclonal antibodies were selected based on their strong reaction against P. salmonis and absence of cross-reactivity with other common fish pathogens. Western blot analysis showed that the antibodies reacted to several antigens of P. salmonis. Immunofluorescence assays revealed that these antibodies reacted with the same specificity to different isolates of P. salmonis obtained from the south of Chile. This panel of monoclonal antibodies represents an important tool to develop simple, rapid, sensitive and highly specific methods for the detection of the pathogen and diagnosis of the disease.

    Notes: Summary Incisional hernia is a common problem seen by the general surgeon, with one alternative for repair being the use of a prosthetic material. This study analyzes the results obtained with an expanded polytetrafluoroethylene patch (e-PTFE) in the management of 53 moderate and large incisional hernias, using a predesigned operating approach. A specific technique was indicated depending on the result of evaluation of the transverse diameter of the hernial orifice and the status and trophy of the rectal muscles and their sheaths. The PTFE prosthesis was used in 30 cases as reinforcement, while in the remaining 23 cases it was placed as the only retention plane. All patients were reviewed a minimum of 6 months and a maximum of 78 months postoperatively, with an average follow-up of 40 months. There were no postoperative respiratory or haemodynamic complications and there was no operating mortality. There was wound infection in two cases (3.7%), and four recurrences were detected (7.5%). On the basis of our clinical experience, we believe that e-PTFE is an acceptable mode of treatment for incisional hernias. With the predesigned operative approach, the infection rate was low with a recurrence rate of 7.5% at a mean follow-up of 40 months.

    Notes: Summary The preoperative diagnosis of inguinal hernias is one of the surgeon's most commonplace duties yet one of the least valued aspects in the treatment of this pathology. The introduction of the laparoscopic technique for inguinal hernia repair may stimulate an interest in this problem. The aim of the study is to analyse the diagnostic accuracy of clinical examination of inguinal and femoral hernias. 278 patients with a possible inguinal and femoral hernia received from the same surgeon a thorough clinical examination and preoperative diagnosis of the type of hernia (indirect or direct inguinal, and femoral). The data obtained were compared to the intra-operative findings. The influence of age, sex and site was studied with regard to the sensitivity of the clinical diagnosis. Direct inguinal hernias accounted for 35% of the total. Indirect inguinal hernias were diagnosed more accurately (85%) than direct (64%) and femoral (39%) hernias. Clinical diagnosis was not influenced by age, sex or site of the femoral hernia. Statistically significant differences were revealed only for inguinal hernias in the group of patients aged over 50 years and with a unilateral site (p〈0.05). Thorough physical exploration should never be underrated in the diagnosis of inguinal hernias as it may help classify hernias with great accuracy.

    Notes: Summary The treatment of inguinal hernias using laparoscopy can be performed without violating the peritoneal cavity using the totally extraperitoneal technique (TEP). This procedure is usually done with general anaesthesia. The objective of this article is to evaluate the general and regional anaesthesia techniques in extraperitoneal laparoscopic surgery for treating inguinal hernias in an outpatient surgery unit. A prospective clinical study of 131 patients with uncomplicated inguinal hernia undergoing surgery using extraperitoneal laparoscopy was completed. Two study groups were established according to the anaesthesia technique used: general (n = 90) and regional (n = 41). We analyzed clinical data (age, sex, associated diseases, prior abdominal surgery, site and hernia type), intra-operative complications (bleeding, peritoneal rupture, subcutaneous emphysema, reconversion rate, haemodynamic stability, respiratory problems and degree of satisfaction), postoperative complications (haematomas, urinary retention, post lumbar puncture headaches, nausea, vomiting and postoperative pain) and recurrence rate. General anaesthesia was used significantly more in the cases of prior infra-umbilical surgery and bilateral hernias (p 〈 0.05). Statistically significant differences were not shown for intra- or post-operative complications. The rate of conversion was higher for general (5.5%) than for regional anaesthesia (2.4%). Recurrence was detected only in the regional anaesthesia group. In conclusion, general anaesthesia is not required for the performance of extraperitoneal laparoscopic inguinal hernia repair; regional anaesthesia is a safe and efficient alternative.

    Notes: Summary Two cases are presented of benign stenosis of the cardia secondary to fibrosis following antireflux surgery in which the patients developed a motor alteration in the esophageal body similar to that of achalasia of the cardia. There was a complete absence of contractions in one patient, which had developed over a long period of time, and a vigorous pattern in the other patient, which had evolved over a short period. In both cases, after surgical treatment of the stenosis, normal motility in the esophageal body returned.

    Notes: Abstract The present paper evaluates the efficiency of Nissen fundoplication as an antireflux technique in a series of 51 patients with different grades of esophagitis. Follow-up time averaged 6.6 years. The evaluation includes a clinical assessment, endoscopic study, and 24-hr pH monitoring. The results reveal an acceptable rate of recurrences, with clinical (9.8%) being less frequent than endoscopic (13.7%) or pH-metric (19.6%) recurrences. Side effects appeared in 37.3% of the patients (mainly inability to belch or vomit, and postprandial fullness), which were mild and transitory in most cases. Nissen fundoplication proved effective in controlling long-term gastroesophageal reflux, as over 80% of the patients presented an excellent-to-good clinical situation, without or with minimal digestive consequences.
