    Notizen: Plasma electrolytes, glucose and insulin concentrations were measured and serial electrocardiograms performed during the intravenous infusion of salbutamol to five women in premature labour. The plasma potassium (mean±standard error) fell from 3·5±0·l to 2·7±0·l mmol/l, and the glucose and insulin rose by 4·2±0·7 mmol/l (72·13 mg/100 ml) and 26·5 mU/l respectively. The maximum fall in potassium, and the rise in glucose and insulin occurred two hours after starting the infusion. No changes in other electrolytes were found. Electrocardiograms showed no signs of hypokalaemia. Highest pulse rates (148·4 beats per minute) occurred at two hours but no arrhythmias were observed.

    Notizen: Twenty seven insulin-dependent diabetics, and six non-diabetic subjects undergoing elective surgery have been studied. Twelve diabetics received continuous glucose-insulin-potassium (GIK) infiion for at least 4 hours after surgery terminated. Six diabetic patients having morning surgery received a proportion of their morning insulin dose with intravenous glucose (25 g) before surgery and the remaining jve operated on in the afternoon received their morning insulin with breakfast. Non-GIK groups were combined and compared with GIK. Postoperative diabetic treatment was the same in both groups. Plasma glucose changes were studied in all patients and other metabolites whenever possible.Mean pre-operative glucose, non-esterjedfatty acid, and 3-hydroxybutyrate concentrations were similar in GIK and non-GIK groups. Four hours postoperatively plasma glucose, and 3-hydroxybutyrate values were lower in the GIK group than in the non GIK group (p 〈 0.05) as were mean plasma non-esterfed fatty acid levels. Plasma glucose concentration was also lower in GIK subjects at 72 hours postoperatively (p 〈 0.01). At other times measured metabolic variables were similar in both GIK and non GIK groups.

    Notizen: In a prospective randomised study in 20 insulin-dependent diabetics who had minor surgery under general anaesthesia we compared the metabolic responses to intravenous glucose-insulin-potassium infusion with those who had conventional subcutaneous insulin administration. The former treatment resulted in lower blood glucose levels both during the infusion period (p 〈 0.05) as well as the entire observation period (operative, first and second postoperative days; p 〈 0.01). More blood glucose values were within the intended range of 5 to 10 mmol/litre in the glucose-insulin-potassium as compared to the conventional group (48% versus 24%; p 〈 0.01). The levels of lactate, 3-hydroxybutyrate, glycerol, alanine, glucagon, insulin and growth hormone did not differ between the two groups. The infusion regimen resulted in better glycaemic control both peri-and postoperatively than the conventional subcutaneous insulin regimen in insulin-dependent diabetic patients who have minor surgery.

    Notizen: SUMMARY 1. Sodium salicylate has been used as a hypoglycaemic agent in the treatment of diabetic patients and severe hypoglycaemia has been reported in children treated with aspirin. The mechanism of this hypoglycaemic action has never been elucidated.2. In the isolated perfused rat liver, salicylate inhibits gluconeogenesis from lactate, alanine and propionate. The degree of inhibition is related to salicylate concentration and occurs with 2 mM (30 mg/100 ml).3. The inhibitory action probably results from the effect of salicylate on oxidative phosphorylation in that there was little or no inhibition of gluconeogenesis from precursors which do not have a high requirement for ATP in their metabolism to glucose.4. In man, lactate and alanine are the main precursors of glucose, and the liver is the main site of gluconeogenesis. These results demonstrate a mechanism for salicylate induced hypoglycaemia in man, and in addition suggest that accumulation of lactate may account for the acidosis sometimes produced by overdosage with salicylate.

    Notizen: Summary Plasma insulin concentrations of insulin-treated diabetic patients must be measured after removal of insulin antibodies, usually by precipitation with polyethylene glycol (PEG). Details of the procedure vary between laboratories; commonly, frozen plasma is thawed and incubated at 37 °C to restore a presumed equilibrium between free and antibody bound insulin before extraction. The present study was designed to investigate methodological factors that could affect the measured free insulin concentration. In normal subjects PEG extraction of globulins did not disturb measurement of insulin concentrations, whether carried out after incubation for 2 h at 37 °C, or storage at -20 °C, in either order. Freezing or incubation of PEG extracts of plasma from insulin-treated patients also failed to disturb the measured concentrations of free insulin. When plasma from patients was incubated for 2 h after storage, a marked scatter (51–272%) of measured results occurred when compared to bedside extraction. This problem was not overcome by buffering with HEPES or storage at a lower temperature (-40 °C). Incubation at 0 °C also severely disturbed the apparent concentrations. Incubation of plasma before extraction and freezing also disturbed the measured result, a problem not corrected by maintaining near physiological pH. Total insulin concentrations measured on acid extracts were not disturbed by any of these manoeuvres. The temperature of centrifugation of blood at the time of venepuncture did not influence the result. Furthermore, when insulin concentrations were rising or falling similar percentage changes were seen over a 30-min incubation of plasma before extraction on the day of venepuncture, suggesting that equilibrium between free and bound insulin is maintained in vivo. We suggest that, for accurate estimation of free insulin concentrations in insulin-treated diabetic patients, immediate centrifugation of blood and extraction of insulin antibodies is mandatory.

    Notizen: Summary Twelve hour metabolic rhythms have been determined in two groups of subjects during combined therapy with a sulphonylurea and phenformin 50 mg twice daily. Subjects with clinical evidence of complications of diabetes showed greater abnormalities in concentrations of blood intermediary metabolites than a group of subjects without complications despite similar mean blood glucose concentrations in the two groups (7.6 mmol/l with complications; 7.3 mmol/l without complications). Mean blood lactate (1.93 mmol/l v 1.39 mmol/l), alanine (0.56 mmol/1 v 0.43 mmol/l), total blood ketone bodies (0.20 mmol/l v 0.14 mmol/l) and several other intermediary metabolites and their ratios were significantly higher in the group with diabetic complications. It is suggested that the differences between the two groups may arise from impaired disposal of phenformin leading to higher blood concentrations in the group with diabetic complications, despite normal liver function tests and plasma creatinine concentration. It is probable that this accumulation of phenformin results in more pronounced effect upon blood glucose and other intermediary metabolites. Thus, the metabolic abnormalities previously reported in patients treated by phenformin alone are also present during combined sulphonylurea and phenformin therapy, and in the presence of diabetic microangiopathy these abnormalities are accentuated.
