    Notizen: Abstract Automutilation of the cornea occurs more frequently than is generally supposed. The patient, particularly at his first visit, certainly will not reveal the cause of the corneal lesion. In two University Eye Clinics 19 patients were seen in whom the diagnosis of automutilation of the cornea could be made. It is important for the ophthalmologist to bear the possibility of automutilation in mind. In 6 of our patients corneal grafts were performed. All these grafts failed because of further automutilation. Corneal transplantation is certainly not indicated; these operations were performed because the patient's true condition had not been spotted in time. Better forms of treatment are tarsorrhaphy or the application of closely occluding bandages. In this way fair results were obtained in 2 cases. It is wrong to force the patient to a confrontation (i.e. to tell him that automutilation is suspected). Confrontation can lead to more severe mental disorders. In all cases psychiatric treatment is absolutely essential. The ophthalmological prognosis for this condition is poor and psychiatric help is quite often refused.

    Notizen: Abstract Over the years we have noticed on more than one occasion a connection between rejection of a corneal graft and emotional stress. We considered this at first to be a coincidence. However, we have now seen 11 patients in whom an acute rejection occurred just after emotional stress. There appears to be a clear relationship between psychological stress and rejection.

    Notizen: Abstract Four cases of melting of the recipient part of the cornea after cornea transplantation are described. Melting of the cornea has been reported in auto-immune diseases as for example: rheumatism and Sjögren's disease. As far as we know it has never been described after keratoplasty. The 4 patients had a history of many years of recurrent herpetic stromal disease. We describe the case histories of the patients and the possible causes of the phenomenon.

    Notizen: Conclusion Many factors influence the value of a tear substitute. Not only its therapeutic value, but also the preservative used and the manageability of the container play a very important role. The preference of the patients was for Liquifilm tears and Duratears eyedrops, for the night Vidisic was the most satisfactory preparation. The preservative benzalkonium chloride should be avoided as far as possible because of its cytotoxic action. The clinical picture of hurricane keratitis indicates that too much of this preservative has been applied to the eye.

    Notizen: Abstract 54 corneas, 49 high risk cases and 5 low risk cases, were grafted with HLA-matched donor material. The importance of antigen compatibilitiy especially in high risk cases is demonstrated. The percentage of irreversible rejections and reversible rejections in this group is markedly smaller as compared with our own “at random” material and the “at ramdom” material of other authors. The follow-up period is 3–21 months.

    Notizen: Abstract Corneal dystrophy following lens implantation is a fairly frequent complication. In our material from the last 2 years (293 corneal grafts) we checked the number of cases of these iatrogenic dystrophies which we had treated. The causes of the dystrophy were investigated and the results with respect to the visual acuity after corneal grafting were reviewed. It appears that the most favourable results are obtained if the corneal graft is performed within 9 months of the onset of the dystrophy.

    Notizen: Abstract Histological examination can differentiate between Fuchs' dystrophy of the cornea and the bullous keratopathy which occurs after cataract extraction, with or without the implantation of an artificial lens. The differences are apparent in Descemet's membrane and the endothelium. In 89% of cases there was agreement between the histological and the clinical diagnosis.

    Notizen: Abstract Six rejected human corneal grafts out of a group of 40 ‘bad-risk corneas’ were systematically studied by light and electron microscopy. Remarkable fine structural similarities in this sample of corneas were observed. Granulocytes, lymphocytes and plasma cells are common in all rejected grafts. Furthermore the epithelial and endothelial linings and the stroma reveal characteristic alterations. The observations indicate an immunological reaction of the recipient to the donor corneas.
