    Notes: Abstract In a review of 77 HIV positive children seen between 1981 and 1990, 32 were diagnosed as having lymphocytic interstitial pneumonitis). Four of the LIP group developed bronchiectasis, a finding not previously reported. The precise factors leading to the bronchiectasis are unclear. All patients had chronically consolidated lung with volume loss. A history of recurrent bacterial superinfection was not noted in any of the cases. With more cases of HIV positive children living longer, bronchiectasis, long known to occur in primary immunologic disorders, will probably be more frequently noted.

    Description / Table of Contents: Summary Question: Language impaired children are at special risk for developing psychiatric problems. In the present study we examined if the Child Behavior Checklist assessed the emotional and behavioral problems in these children and if the questionnaire could be used as a screening instrument in populations of language impaired children. Methods: The behavioral and emotional problems of a sample of 83 language impaired children were assessed using clinical psychiatric interviews and observations as well as a parent questionnaire. Two case definition procedures based on the Child Behavior Checklist were presented (The Total Behavior Problem Score, presence of clinically relevant symptoms) and crosschecked with the clinical diagnosis. Results: 66 children had a psychiatric diagnosis on axis I of the multiaxial classification scheme. Psychiatric diagnoses were mainly Attention Deficit Disorder with or without Hyperactivity and Emotional Disorder. Children with a psychiatric diagnosis had higher raw scores on the Total Behavior Problem Score and had more clinically relevant symptoms than the children without a psychiatric diagnosis. Both case definition procedures were checked for their sensitivity, specificity and false classification rate. Conclusions: The Child Behavior Checklist can be used as a screening instrument in populations of children with specific developmental speech and language disorders. In addition to a raw score above 40 on the Total Behavior Problem Sore, the presence of specific symptom constellations necessitates a careful psychiatric evaluation of these children.
    Notes: Zusammenfassung Fragestellung: Sprachentwicklungsgestörte Kinder haben ein erhöhtes Risiko, psychiatrisch zu erkranken. In der vorliegenden Untersuchung wird überprüft, ob ein Fragebogen (Child-behavior-Checkliste) die Verhaltensauffälligkeiten der Kinder zuverlässig erfaßt und im Rahmen eines Screenings eingesetzt werden kann. Methode: Die Verhaltensauffälligkeiten von 83 sprachentwicklungsgestörten Kindern wurden sowohl durch eine klinisch-psychiatrische Diagnostik als auch mittels der Child-behavior-Checkliste erfaßt. Der Fragebogen war die Grundlage für 2 Falldefinitionsmodalitäten (Gesamt-Score gegenüber einer Bewertung von behandlungsbedürftigen Einzelsymptomen), die der klinisch-psychiatrischen Diagnostik gegenübergestellt wurden. Ergebnisse: Anhand der klinisch-psychiatrischen Diagnostik wurde bei 66 Kindern eine Diagnose auf Achse I des Multiaxialen Klassifikationsschemas gestellt. Schwerpunktmäßig ließen sich Aufmerksamkeits- und Hyperaktivitätsstörungen sowie emotionale Störungen nachweisen. Bei 17 Kindern wurde keine psychiatrische Diagnose gestellt. Es zeigte sich, daß die Kinder mit einer psychiatrischen Diagnose im Gesamt-Score der Child-behavior-Checkliste deutlich höhere Rohwerte hatten als die Kinder ohne psychiatrische Diagnose. Die psychiatrisch auffälligen Kinder hatten ebenfalls mehr behandlungsbedürftige Symptome im Fragebogen als die psychiatrisch unauffälligen Kinder. Die beiden Falldefinitionsmodalitäten wurden bezüglich ihrer Sensitivität, Spezifität und Fehlklassifikationsrate überprüft. Schlußfolgerungen: Die Child-behavior-Checkliste ist geeignet, um Verhaltensauffälligkeiten bei sprachentwicklungsgestörten Kindern zu erfassen. Neben einem Rohwert über 40 im Gesamt-Score des Fragebogens müssen im Rahmen eines Screening-Verfahrens auch bestimmte Symptomkonstellationen Anlaß zu einer genauen psychiatrischen Abklärung geben.

    Notes: Abstract Localization of the site of atresia in imperforate anus has classically been carried out by the “upside down” radiograph identifying the blind rectal end, pubic bone, and coccyx on lateral X-rays. This method is often inaccurate because of difficulty in identifying the bony structures and absence of air in the blind rectum. We have studied 6 neonates with CT scans of the pelvis: two males with “high” imperforate anus, one male with a “low” imperforate anus, a female with a cloacal anomaly, a normal stillborn, and a baby with myelomeningocele with a normal anorectum. The CT clearly and accurately identifies all bony structures and pelvic muscle aand determines if bowel is present below the pubococcygeal line, or if it ends above it. In addition, the CT software can generate sagittal and coronal (frontal) reconstructed views from the axial sections to identify the blind rectal end and anus and measure the distance between them. Bowel contents (air and meconium) are distinct other pelvic structures, and CT can muscle and other pelvic structures, and CT can identify these. Identification of the blind pouch by CT is not meant to replace the conventional radiographic evaluations, but may be useful in difficult cases. CT has the potential to identify the level of the blind pouch immediately after birth, without waiting for air to reach the blind rectal end.

    Notes: Summary The effects of 6 weeks of thrice weekly training on glycaemic control were assessed in 20 sedentary Type 2 (non-insulin-dependent) diabetic patients and 11 control subjects matched for previous physical activity. Maximal oxygen uptake was lower in the diabetic patients than in control subjects before training (26.2±1.1 versus 32.6±1.7 ml·kg-1·min-1; p〈0.001). Glycosylated haemoglobin levels decreased in the diabetic patients during the training programme (12.2±0.5 to 10.7±0.4%; p 〈 0.02). Oral and intravenous glucose tolerance determined 72 h after the last exercise period showed only minimal improvement. Plasma glucose levels were, however, significantly lower at 12 h than 72 h after exercise in eight subjects tested at both time points. These data suggest that an exercise programme can produce a significant decrease in glycosylated haemoglobin levels in Type 2 diabetic males probably due, in great measure, to the cumulative effect of transient improvements in glucose tolerance which follow each individual period of exercise.
