    Notes: Abstract  There has been much speculation in the literature about the effect of the prospective payment system (PPS) on the home care industry but few reports of systematic investigation. Clearly, comprehension of client characteristics is essential for categorizing home care clients into the correct Home Health Resource Groups, for allocating resources appropriately, and for implementing necessary organizational changes to accommodate clients who require variations in the amount of resources predicted by the PPS. The purposes of this study were to compare the characteristics of home health clients that resulted in a financial gain or a financial loss for one agency under the PPS with those of the interim payment system. A secondary data analysis of 140 cases compiled by one not-for-profit hospital-affiliated home care organization was completed. Results suggested that client characteristics of the projected loss group had more recertifications on admission and at the end of the 60-day episode, a longer stay (3 weeks), and more visits, particularly from skilled nursing. Comprehension of client characteristics and the subsequent ability to adjust for the proper case mix will be crucial if home care agencies are to remain financially viable under the PPS. Replication of this project needs to be done using more variables and multiple sites.

    Notes: Abstract Home health care costs have increased more than any other health care service covered by Medicare, and new methods of payment by Medicare are being introduced. A prospective payment system would replace the per visit rate now paid to home care agencies by Medicare. In order to remain viable under a new payment system, home health care agencies will need to be efficient in the delivery of home care services and still demonstrate effective patient care. The intent of this descriptive study was to determine norms of resource utilization for the congestive heart failure (CHF) patients admitted and discharged from a not-for-profit home health agency (HHA). Forty agency records were retrospectively reviewed using the Resource Utilization Inventory to collect the characteristics and resource utilization of the sample group. The CHF patients were older than most home care clients and had chronic health problems. Because of these chronic health problems, over half of the study population had caregivers so that the clients could remain in their own homes. Most clients clearly demonstrated the need for skilled nursing or home health aide visits after being hospitalized for an acute CHF episode. Less than half of the study group were discharged from the HHA as improved, and frequently were discharged to another healthcare facility. These results provide a beginning direction for profiling the CHF patient's consumption of resources for setting prospective reimbursement rates.

    Notes: Field studies along the southeastern shore of Trout Lake, Wisconsin, documented the presence of downward hydraulic gradients in a known discharge area as well as an anomalous distribution of seepage to the lakebed which deviates significantly from the generally accepted dogma that ground-water seepage rates decrease exponentially with distance from shore. A numerical ground-water model facilitated identification of the hydrologic control, namely the presence of a unit of high hydraulic conductivity, that accounts for the anomalous data, and is important for understanding the dynamics of the flow system. Field data including seepage measurements, visual inspection of lakebed materials and springs, and information obtained during drilling, indicate that a lens of coarse-grained material intersects the lake. However, the significance of the coarsegrained material was not fully appreciated until a ground-water model was used to simulate the flow system. The model indicated that the presence of the coarse-grained lens has a marked effect on the flow pattern in the nearshore area causing downward hydraulic gradients which divert ground water into the lens and cause the occurrence of a localized high-seepage area offshore where the lens intersects the lake. The numerical model predicts the anomalous seepage distribution noted in the field and the downward hydraulic gradients demonstrating that numerical models are practical tools for interpreting field data and for use in hypothesis testing.

    Notes: Ground-water flow models developed for the flow systems around Bass Lake and Nepco Lake, Wisconsin, show that models can be useful tools for estimating lake seepage rates. A two-dimensional profile model for Bass Lake illustrates that the ratio of horizontal to vertical hydraulic conductivity of the aquifer around the lake is related to both the magnitude of vertical hydraulic gradients near the lake and the distribution of seepage from the lake as a function of distance from shore. The model also shows that it is important to know the vertical hydraulic conductivity of littoral lake-bed sediments to estimate lake seepage rates.A three-dimensional model constructed for an unusual ground-water flow system around Nepco Lake provides a more realistic simulation of the flow system than does a comparable two-dimensional model. Both models were useful for identifying additional data that could be used to quantify the ground-water portion of the lake's water budget more accurately. It is suggested that a combination of two-and/or three-dimensional models could be used to estimate seepage rates at many lakes for which sufficient data exist.
