    Notes: Purpose The purpose of this clinical pilot study was to evaluate the effect of a tricyclic antidepressant, amitriptyline, on pain-intensity level and level of stress in bruxers.Materials and Methods In a randomized, double-blind, crossover experimental design, 10 subjects received active (amitriptyline 25 mg/night) and inactive (placebo 25 mg/night) medication, over a period of 4 weeks.Results The administration of amitriptyline for 4 weeks did not significantly (p 〉 .05) reduce pain intensity. However, it significantly (p 〈 .05) reduced the level of stress perception.Conclusion The results of this limited study do not support the administration of small doses of amitriptyline over a period of 4 weeks for the management of pain resulting from sleep bruxism. However, the results support the administration of small doses of amitriptyline for the management of the perception of stress levels associated with sleep bruxism.

    Notes: Clinicians have long expressed concern about the accuracy of the Gothic arch tracing for recording centric relation in edentulous patients. With the use of dental implants to assist in retaining complete dentures, the problem of inaccurate recordings, made for patients without natural teeth, can be significantly reduced. This article presents a technique that uses healing abutments to stabilize the record bases so that an accurate Gothic arch tracing can be made.

    Notes: Objective To estimate the existence and degree of fetal accumulation of acid during end-stage bradycardia as reflected by the base deficit. This may set a criterion for proper intervention during labour.Setting Maternity unit of the Tel Aviv Medical Centre.Subjects Forty-three consecutively born term infants whose mothers were delivered by vacuum extraction were analysed 27 because of end-stage bradycardia and 16 controls whose mothers were delivered electively because of maternal indications.Main outcome measures Analysis of umbilical arterial cord blood for pH, Pco2, and base deficit. The base deficit was compared between the groups using the two-tailed Student's t test, and was correlated with variables of fetal heart rate monitoring using the Pearson correlation coefficient.Results The base deficit was greater in newborns who had end-stage bradycardia than in controls (11.02 vs 5.01, P 〈 0.0001). The duration of loss of short term variability in fetal heart rate during end-stage bradycardia correlated positively with the base deficit (r= 0.8, P 〈 0.0005). Conversely, the time until the loss of short term variability during end-stage bradycardia correlated negatively with the base deficit. The length and the depth of the bradycardia and their product, had a weaker correlation with the base deficit.Conclusions End-stage bradycardia, which presumably reflects fetal hypoxia, is associated with acidaemia in the umbilical artery at birth in some fetuses. The fetuses who are predisposed to acidaemia, as reflected by an increased base deficit, are those who lost their fetal heart rate variability during end-stage bradycardia for more than 4 min or started to lose this in less than 3 min from the beginning of the end-stage bradycardia. Operative vaginal delivery should be reserved for these indications.

    Notes: In this review, we conceptualize exposure-based treatment as a learning experience. With this approach, optimizing treatment is a matter of optimizing memory for new learning. Given that perspective, we discuss the implications of a “new theory of disuse,” proposed by Bjork and Bjork (1992) to capture the storage and retrieval dynamics that characterize human memory. The theory distinguishes between the storage strength and retrieval strength of learned representations and provides a framework from which we derive a number of manipulations that have the potential to improve the long-term effectiveness of cognitive behavioral therapy. Implications for treatment of specific fears are described in detail, with additional discussion regarding treatment for other emotional disorders.

    Notes: Metal-free tooth-colored restorative systems for fabricating indirect intracoronal and extracoronal restorations have been developed because of patients' demand for superior esthetics. During the past decade, long-term clinical studies have demonstrated the success of many of these types of restorations in terms of function and esthetics. Ceramic veneers, all-ceramic crowns, ceramic onlays, and composite-resin inlays are becoming an integral part of contemporary clinical practice. The constant evolution in bonding techniques, adhesive systems, and cements plays a major role in the success of these metal-free restorations. In addition, home-bleaching procedures have become an integral part of the esthetically oriented dental practice. This article discusses the rationale, scientific basis, and implementation of a new undergraduate preclinical esthetics course for second-year dental students at the Louisiana State University Health Sciences Center School of Dentistry.CLINICAL SIGNIFICANCEChanges in the undergraduate dental curriculum are ultimately necessary to provide undergraduate students with the adequate theoretic knowledge and clinical skills they require to face challenging practice demands and patients' requests for esthetic restorations. The implementation of the Preclinical Esthetics course at Louisiana State University School of Dentistry gives undergraduate dental students a foundation for restoring patients with indirect tooth-colored restorations, such as composite-resin inlays, ceramic onlays, ceramic veneers, and all-ceramic crowns, and vital bleaching procedures.

    Notes: The use of all-ceramic fixed partial dentures is still controversial, although materials for ceramic frameworks have been introduced into the market and new materials and techniques and technologies for their fabrication are constantly evolving. In the quest to provide scientific data evaluating these new systems and to provide clinicians with guidelines for their use, in vitro studies followed by clinical ones have been and are currently performed. Although in vitro strength studies provide both clinicians and researchers with useful information for selecting ceramic materials and techniques for further research, they might not directly predict or reflect clinical performance. To enhance their clinical relevance, in vitro studies should better mimic oral conditions, such as cyclic loading and contact stresses, tooth mobility, temperature changes, and chemically assisted degradation. Such in vitro studies simulating oral conditions will provide clinically relevant information. However, clinicians must rely on the results of properly designed clinical studies to determine the efficacy of all-ceramic fixed partial dentures before prescribing such treatment to their patients. This “Critical Appraisal” continues a discussion of the in vitro studies. Part III will discuss the clinical studies on the clinical performance of all-ceramic fixed partial dentures.
