    Notes: This study examines the hypothesis that cartilage-derived morphogenic protein-2 (CDMP-2) can improve tendon healing after surgical repair. We have previously found improved tendon healing by applying CDMP-2 in models for conservative treatment with mechanically loaded Achilles tendon defects in rats and rabbits. In this study, the patellar tendon was unloaded by patello- tibial cerclage and cut transversely in 40 rabbits. Two hours post-operative, the rabbits received a dose of 20 μg of CDMP-2 or vehicle injected into the hematoma. Specimens were harvested after 14 and 28 days and evaluated by biomechanical testing, radiography and histology. At 14 days, CDMP-2 caused a 65% increase in force at failure, a 50% increase in ultimate stress and a 57% increase in stiffness, as compared with controls. There was no effect on callus size. At 28 days, no differences between the treatment groups could be demonstrated. No bone or cartilage was found in any tendon or regenerated tissue at any time point. Thus, early tendon repair can be stimulated by CDMP-2 in an unloaded model. These results suggest that CDMP-2 might be of interest for clinical use as a complement to surgical treatment of tendon ruptures.

    Notes: Bone repair is controlled by signalling proteins as well as mechanical stimuli. Bone morphogenetic proteins (BMPs), implanted or injected into a fracture site, can enhance bone healing, and so can mechanical stimulation. Possibly, mechanical signals can alter BMP expression, but furthermore, tissue differentiation initiated by an implanted BMP could be guided into the required direction by mechanical signals. Mechanical stimulation also has profound effects on tendon repair. We studied the effects of 3 different BMPs in a rat tendon repair model. One of these, osteogenic protein 1 (OP-1) induced bone formation in the tendon at the expense of mechanical strength and thus overrode any mechanical control system. On the other hand, cartilage derived morphogenetic protein 1 and 2 (CDMP-1 and -2) both made the tendon callus bigger and stronger at an early time point.

    Notes: Abstract In previous rabbit chamber experiments, lipid extraction has been shown to increase bank bone incorporation, as measured by scintimetric activity at 3 weeks. In the present study, the new bone ingrowth distance was measured by histomorphometry at 6 weeks using a titanium chamber model in rats. By insertion into bilateral bone conduction chambers, frozen grafts were compared with grafts that had been processed by lipid extraction. To evaluate the effects of lipid extraction further, the group of 26 rats was divided into three subgroups according to MHC haplotype, namely a heterogeneous group (outbred Sprague-Dawley rats), a mismatched group, and a syngeneic group. In the total material, defatted grafts showed a 58% greater new bone ingrowth distance and a 31% higher scintimetric activity over controls. The effect of defatting was not shown to be due to immunologic factors. In general, rats with a lower capacity to incorporate bone grafts showed a larger positive effect of defatting.

    Notes: Summary Implants of demineralized bone matrix induce new bone formation. In order to estimate the possible clinical usefulness of this phenomenon, autologous cancellous bone grafts were compared with composite grafts of bone matrix and marrow. Cancellous bone from the tuber ischii of the rabbit was transplanted to a preformed radial defect in the same animal. On the opposite side, a similar defect was filled with a mixture of either allogenous or autogenous bone-matrix particles and autogenous bone marrow. After 25 days, calcium 45 was injected intravenously. Three days later the animals were killed. Standardized segments of the rabbit's forearms, containing the middle of the defect, were cut out, ashed, and analyzed for 45Ca activity. No side difference in 45Ca deposition was found. The callus ash weight of the allogenous matrix-transplanted side was approximately 60% of that of the cancellous bone side. This side difference of ash weights corresponds to the estimated initial mineral content of the cancellous graft. Nontransplanted defects had very low ash weight and 45Ca activity. Thus, in the rabbit, composite grafts of bone matrix and marrow produce a bone yield comparable to that of cancellous bone.

    Notes: Summary The influence of systemic treatment with biosynthetic human growth hormone on bone formation was studied, comparing rabbit radial bone defects grafted with demineralized autologous bone with contrallateral defects grafted with a composite including bone marrow. At 14 and 28 days after operation the bone-forming process was evaluated by roentgenography (planimetry included), and by scintigraphy after injection of 99mTc-labeled dicarboxypropane diphosphonate. At 28 days autoradiography was also performed. The roentgenographic and scintigraphic examinations revealed no difference between the grafted sides at either 14 or 28 days postoperatively. The elimination, after 14 days' growth hormone treatment, of a previously found difference between the marrow- and the nonmarrow-grafted sides might be an indication of accelerated osteogenesis at the beginning of the healing process.

    Notes: Summary Bovine bone chips (Surgibone®) were used in occipito-cervical fusion in nine patients with atlanto-axial instability due to rheumatoid arthritis. The patients were examined with CT 12–15 months after surgery. Graft resorbtion was observed in one patient. The other 8 patients showed preserved grafts, in most cases the grafts appeared to be in contact with the underlying bone. One patient was revised, and at the grafted site a bony bridge was found. In conclusion, the use of bovine chips in posterior occipitocervical fusion will not lead to predictable bone union. However, there seem to be exceptions to that rule.
