Notes: Objective To assess the occurrence of disappearance of one or more of the fetuses in pregnancies which start as multiple gestation.Design Observational study.Setting Infertility section, Rambam Hospital, Haifa.Subjects 88 women with multiple gestations, established after ovulation induction (54 twin, 26 triplet, five quadruplet, and three quintuplet) and diagnosed by trans-vaginal ultrasound at 5–6 weeks, in all of whom absorption of at least one gestation sac was detected at follow-up ultrasound scan.Interventions Follow-up by serial transvaginal and later abdominal ultrasound scan throughout pregnancy. Results Of the 54 twin gestations, 51 ended in the birth of a singleton and three in miscarriage. Of the 26 pregnancies starting as triplets, 12 ended in singleton births, 12 in twins and two miscarried. The five quadruplet gestations resulted in one singleton birth, one set of twins, two triplets, and one ended in late miscarriage. Of the three quintuplet pregnancies, two resulted in the birth of triplets, one of them after spontaneous, the other after iatrogenic fetal reduction. In the third quintuplet pregnancy, one fetus vanished spontaneously and another was subject to iatrogenic reduction, two fetuses survived and were liveborn. Of the 221 fetuses identified 107 (48%) vanished spontaneously. Conclusion Iatrogenic fetal reduction should be delayed until 12 weeks gestation in quadruplet or higher multiple gestations, but is probably not indicated in twin and triplet gestations.

Notes: Abstract Tubo-ovarian abscesses are considered a problem only during the childbearing years because of their rarity in the postmenopausal age group. Three case reports are described, in which pelvic abscesses were diagnosed in elderly women and necessitated surgical intervention. The early recognition and surgical treatment of pelvic abscesses is mandatory in the postmenopausal woman, who often is a poor-risk patient. Nothing is to be gained by delay of definitive treatment, and the risk of serious deterioration is obvious. Therefore, the gynecologist and surgeon should be alert to this uncommon diagnosis in the elderly woman and be prepared for early surgical intervention.

Notes: Abstract It is now general professional consensus that to develop a good human sexuality curriculum it is mandatory to first study the future target population. Such a study was done on a national basis at the four medical schools in Israel, concentrating on the senior students. The data on background, sexual experience, knowledge, and attitudes were analyzed and compared to findings of similar studies in other countries. This article reviews the results on background and sexual experience, also trying to analyze logical hypotheses of possible links between variables. Most Israeli senior medical students are unmarried Jewish Ashkenasi males, with a rather small proportion of females, Sephardic-originated Jews, Moslems, and Christians. Their sex education during childhood originated mostly from peers and literature, but was almost completely lacking at school. Masturbation started early, being more frequent for males. The majority of both sexes had dated, had gone steady, and had had sexual intercourse frequently with at least two partners. Only 5.4% of males had never experienced coitus. Intercourse with prostitutes was infrequent. The students' background has direct influence on their early sexual knowledge and experience. This in turn influences self-ratings on sexual matters. This is further correlated with sexual behavior.

Notes: Abstract The Perceived Personal Control Model (PPC), a novel crisis intervention model for Ceasarean Birth (C/B) populations, is used at Rambam Medical Center by medical caregivers trained in the principles of the crisis intervention model. This model tackles risk factors in the environment by manipulating such situational variables as natural and organized support systems, information, sharing the decision making process, and developing task oriented activity geared to enhance emotional, cognitive, and behavioral control. The specific objective of the intervention model is to help the C/B mother to accomplish the specific psychological tasks. The general objective is to develop a prototype model for other high risk populations. The PPC model with respect to a C/B population is discussed.

Notes: Abstract The study of mothers of premature babies allows for the formulation of the specific psychological tasks that make for adaptive or maladaptive coping patterns. In line with a novel theoretical model of crisis denoted as the Perceived Personal Control Model (PPC), a preventive intervention program was implemented by medical caregivers trained in the underlying principles of the PPC model. That model as well as Social Action to facilitate the accomplishment of the adaptive psychological tasks of the target population are discussed.
