    Notes: Abstract Three members of a family, father, daughter, and son, with the basal cell nevus syndrome are presented. A very unusual manifestation of widespread cyst-like osteolytic lesions in all the tubular bones was observed in the father, together with osteoblastic spotty “osteopoikilotic” lesions in the skull and the mandible of the same patient. Cyst-like osteolytic lesions have been described previously in this syndrome, mainly in the phalanges. We believe that such lesions can occur in any bone.

    Notes: Abstract A case is reported of benign lymphoid hyperplasia in the stomach and proximal duodenum in a 12-year-old girl. This condition is characterized by an abundance of lymphoid follicles in the mucosa which produces the characteristic radiological and endoscopic appearance of mucosal nodularity. It is rare in the stomach and rare in children. The immunoglobulins were normal in our patient.

    Notes: Abstract Necrotizing enterocolitis (NEC) is usually considered to be a neonatal disease, and is rarely described beyond the newborn period. During the last 15 years, 19 infants from the Negev region, Israel, with NEC were beyond the neonatal age group (range=34–616 days, median-90 days). Of this group only 16% were born prematurely, and only 16% had perinatal or neonatal pathology. Diarrhea and dehydration preceding NEC were common as was malnutrition. Bacteremia was detected in 42%. The mortality rate was 90%. Malnutrition, supposedly contributed to the formation of NEC and to the high mortality rate among these infants.

    Notes: Abstract Discitis is an inflammatory process of the intervertebral disc space occurring in the pediatric age range. Its etiology is usually unknown; staphylococcus aureus is the most common microorganism present in the few cases with positive bacteriological cultures. The clinical presentation can mimic medical or surgical emergencies and thus unnecessary investigations or surgical procedures are frequently performed. Irritability, refusal to sit or walk, low grade fever, high erythrocyte sedimentation rate and normal leukocyte count are the most common clinical findings. The disease is usually self-limited and has a benign course. Awareness of the physician is the key for the diagnosis which can be established by nuclear imaging procedures and radiological studies, yet, this disease is not described in standard pediatric textbooks. Eight cases of discitis in children affecting the dorsal, lumbar and cervical spine were diagnosed and treated in two medical centers in Israel. The purpose of this report is to focus the attention of physicians caring for children on this entity.

    Notes: Abstract Two children, aged 3 1/2 and 5 1/2 years, are described. Both developed pulmonary oedema (PE) following a short episode of choking on a sweet and an orange, respectively. On admission diagnosis was made by chest X-ray. One child was asymptomatic despite PE while the other showed only mild respiratory distress. Both children recovered spontaneously and chest X-rays showed a return to normal within 24 h. The mechanism of PE production is discussed. It is suggested that oedema formation occurs during the obstruction and that it is due to hypoxia and the severe negative pleural pressure resulting from attempts to inspire against the obstructed airway. both hypoxia and severe negative pleural pressure cause an increase in pulmonary capillary pressure and transduration of fluid across the capillary membrane.

    Notes: Abstract Bronchial mucoepidermoid tumours (MET) are rare in children. Only 11 cases have been reported in the literature, all of them of the low grade type. A cure is achieved by complete surgical removal of the tumour and bronchoscopy is only used for detection and biopsy. The case of a 12-year-old boy with MET in the bronchus intermedius posed a therapeutic dilemma. In this case, non-intentional removal of the tumour was achieved by bronchoscopy and the patient had no further surgery. Repeated bronchoscopies failed to show recurrence of the tumour after 2.5 years.

    Notes: Abstract To assess residual lung damage after a single insult early in life, we studied 14 asymptomatic children, 10 years after an episode of kerosene pneumonitis. Immediately after kerosene ingestion all patients developed pulmonary symptoms. Eight had abnormal chest radiographs (group 1) whereas in six there were no radiographical changes (group 2). The average age at follow-up was 11.4 (range: 9.4–13.2). Lung volumes, expiratory flow-rates and their density dependence were measured and compared to an age, sex, and a height-matched control group. The volume of isoflow (Viso $$\dot V$$ ) was significantly higher in patients in group 1 (27.75%±6.7) when compared to group 2 (5.48%±3.56) or controls (5.63%±2.31) (P〈0.02). The mean Δ $$\dot V$$ max 50 was 13.5%±3 for group 1, and 27.9%±4.11 for group 2 (P〈0.005). Airway reactivity assessed by an exercise test was normal in all children. Four of the eight patients from group 1 still had abnormal chest radiographs 10 years after kerosene pneumonitis. Thus, subclinical, prolonged small airway abnormalities after kerosene pneumonitis seem to be related to the severity of the acute insult.

    Notes: Abstract A 22-month-old girl with the syndrome of hypoventilation, pulmonary hypertension, cor pulmonale and pulmonary edema due to adenoidal hypertrophy is described. Adenoidectomy resulted in relief of all symptoms and signs within 24 h. Hemodynamic study using pulmonary artery catheter showed that the pulmonary artery pressure returned to normal 48 h after relief of the obstruction. The normal left ventricular end-diastolic pressure, measured throughout the period of obstruction, in the presence of severe pulmonary edema, could suggest a non-cardiogenic “low pressure” pulmonary edema. However, the highly negative pleural pressure which existed during upper airway obstruction indicated an elevation of transmural left ventricular end diastolic pressure (compared to pulmonary wedge pressure) and thus, suggested that the pulmonary edema in this syndrome is secondary to both — right and left heart failure.

    Notes: Abstract Direct coronal plane CT demonstration of the tracheobronchial and pulmonary anatomy is possible. Large gantry openings easily accommodate patients in a sitting or semiprone position without the need for attaching a seat device to the table top. A technique is described for examining the patient using Elscint 1800 and 2400 CT units, with a net gantry angulation of 60–80° to the chest, with resulting good quality direct coronal CT imaging. Our experience includes more than a hundred pediatric patients that have been examined, displaying diverse tracheobronchial, pulmonary and thoraco-abdominal lesions. The technique is safe and comfortable to the patients and is easy to perform.

    Notes: Abstract A rare case of accessory ectopic thymus in the right upper posterior mediastinum in a 40-day-old infant is described. The clinical presentation was that of tracheal obstruction. The compression and displacement of the trachea and the identification of the mass was enabled thanks to the high KV filtration technique. The mass was further identified by ultrasonography. Surgical removal of the accessory thymus relieved the tracheal obstruction.
